Congenital heart disease (CHD) is one of the most common congenital defects in neonates. The incidence rate of congenital heart disease in China is around0.7%to0.8%, with about150thousands such new-born babies every year. Shown by the statistics by national Women’s and children’s monitoring report, neonate death caused by congenital heart disease has become the primary cause. In China, incidence rate of congenital heart disease is higher in the rural than in the urban; however, it is the rural that cannot afford the vast medical expenses.Patients with congenital heart disease increasing year by year to cause the attention of the whole society, the ministry of health, public welfare organizations began to raise money to help patients in rehabilitation project. However, in traditional perspective of medical treatment, medical staffs usually focus only on patients’physical problems; seldom do they pay attention to the mental level of patients and their families. This paper mainly employs a method of group work. Case interview is used when group members are recruited. Then the crew makes a survey about patients’ families’ mental states and problems, and, according to the problems, makes an assessment, and formulates reasonable group plan to help the patients and their families do psychological counseling, besides, the crew also provides social support service and network. Through group work, families of patients can help and encourage each other, which lead to the formation of a social support chain; furthermore, it prompts the ability of the families to look after their patients in order to get them rehabilitated.This paper consists of seven parts. First, it is the introduction. It mainly expounds the background of group work plan a d the causes of the design. The danger of congenital heart disease makes the families of the patients worried; the group work is made according to the actual need of them. The second part introduces theories for designing the service plan for group work, and makes an introduction of main definitions, so that group work can begin in full swing with theories support of superior perspective, energization resilience and social support theories; the third part mainly introduces the goals and importance of group work service. On one hand, foundation is laid for the development of group work; on the other hand, it reveals the necessity of this work. The forth part is about the primary analysis of group work service plan. It is begun with the basic information, essential characteristics of the plan and the recruit of the members of the groups. The fifth part is the concrete implement of the plan, a social support system is formed through five activities of psychological counseling for the families of the patients. The sixth part mainly introduces the main problems and the according measures in the process of implying the plan, effective measures are made to ensure the group work begin smoothly. The last part makes an assessment and conclusion of group work service plan, with the approaches, feedback and effects included, the plan is concluded to make a pavement for later work.
