BackgroundAsymptomatic glomerular hematuria lacks prominent clinical symptoms and is detected during physical examination. In the past, asymptomatic glomerular hematuria was believed to be a benign process, however, with the development of medical research, the vast majority of scholars now believe that the disease could ultimately develop into chronic renal failure. This disease commonly occurs in children and young people, the incidence of men more than women. It is generally believed that this disease is an immune-mediated inflammatory disease. When an infection occurs, antigen-antibody complexes are formed. The deposition of immune complexes in the glomerular mesangial area stimulates mesangial cells to secrete inflammatory media tors including cytokines, growth factors, chemokines etc., thus damaging glomerular basement membrane filtration barrier, so that red blood cells across the filter membrane and by extrusion deformation, lead to glomerular hematuria. Traditional Chinese Medicine believes that the pathogenesis of asymptomatic glomerular hematuria is as follows:spleen-kidney qi and Yin deficiency, in comb in at ion with exogenous wind-heat or damp-heat evil, can lead to kidney damage, thus causing improper blood flow which then results in hematuria. However, the wind evil is the key to this process. The invasion of wind evil and penetration to the kidney meridians results in injuries which cause hematuria; or the wind evil can directly penetrate the kidney, and due to wind being a yang pathogen, and wind by nature tends to cause opening and discharging, the wind evil lurking in the kidney can penetrate and damage kidney membranes and its blood vessels, thus causing hematuria. Qufengtang is composed of xixiancao, chuanshanlong, and jixueteng, with functions of dispelling wind and nourishing blood. This study evaluated the functions of qufengtang by observing the curative effect of qufengtang in the treatment of asymptomatic glomerular hematuria, to provide effective treatment methods for patients with asymptomatic glomerular hematuria.Object i veTo observe the efficacy of qufengtang in the treatment of asymptomat ic glomerular hema tur ia using TCM methodology and principles, with the aim of reducing hematuria and delaying the deterioration of renal function.Methods and Results30patients with asymptomatic glomerular hematuria were given Qufengtang. We analyzed the change in urinary sediment microscopy red blood cell count, immunology index (IgA, IgG, IgM, IgE), renal function, and TCM symptoms both before and after treatment, using a statistical method to describe the treatment results. Results showed that the urinary sediment microscopy red blood cell count decreased significantly after treatment, with significant difference (P<0.001). According to the curative standard of urine red blood cell count:0cases of clinical remission,10cases with significant improvements,11cases with mild improvements,9cases with no improvements, significant efficiency was33.3%, the effective rate was36.7%, the total effective rate was70%. Serum I gA levels before and after treatment are also statistically significant(P<0.05).ConelusionsAsymptomatic glomerular hematuria has a high recurrent rate and is difficult to treat, causing great psychological pressure in patients. If treatment is delayed, it can eventually develop into chronic renal failure. The TCM syndrome of asymptomatic glomerular hematuria is wind evil attack on the kidney. Qufengtang has functions of dispelling wind and nourishing blood, and is proven clinically effective in improving abnormal urine test results, reducing or eradicating centrifugal microscopic examination of the urine red blood cells. In addition, Qufengtang can also reduce serum I g A levels, thus implying that Qufeng tang may play an immunomodulatory role.
