| BackgroundIsolated glomerular hematuria refers to a kind of disease that exclusion of other renal hemorrhage factor, blood comes from the kidneys. The main manifestations are microscopic hematuria or gross hematuria, without obvious edema, hypertension or large number of proteinuria. At the same time, renal function remains normal. It is also known as asymptomatic hematuria, which is one of the most significant clinical manifestations of kidney disease. In the past, isolated glomerular hematuria was believed to be a benign process, however, with the development of medical research, the vast majority of scholars now believe that the disease could ultimately develop into chronic renal failure. It is generally believed that this disease is an immune-mediated inflammatory disease. When an infection occurs, antigen-antibody complexes are formed. The deposition of immune complexes in the glomerular mesangial area and damage glomerular basement membrane filtration barrier, so that red blood cells across the filter membrane and by extrusion deformation, lead to glomerular hematuria. At present, there is no effective treatment in the western medical science. Traditional Chinese medicine considered the disease as blood urine and its pathogenesis summed up as"the virtual scale real", which means the deficiency of spleen and kidney. according to the clinical experience, Tutor Wang found that the wind evil plays an irreplaceable role in the development during the process. The invasion of wind evil and penetration to the kidney meridians results in injuries which cause hematuria. By this, tutor Wang came up with "Qufengtang " to cure this kind of diease and has obtained certain effect. Qufengtang is composed of xixiancao, chuanshanlong, and jixueteng, with functions of dispelling wind and nourishing blood. This research is to observe the clinical efficacy of "Qufengtang"in the treatment of Isolated glomerular hematuria by exploring the analysis of red blood cells evaluating the clinical symptoms before and after treatment. Meanwhile providing theoretical bases for clinical treatment of TCM and dfficacy of isolated glomerular hematuriaObjectiveTo observe the efficacy of qufengtang in the treatment of isolated glomerular hematuria by using TCM methodology and principles, with the aim of providing more ideas and treatments to reduce hematuria relieve symptoms and protect renal function.MethodsA self-control study was done over a 2-month period on 30 patients who have been treated in our hospital from Apr.2014 to Feb.2 2015. Full blood count, liver function, kidney function, and immunology test were conducted and recorded both before and after treatment; urine test was conducted and recorded at two-week interval during treatment; symptoms of TCM were conducted and recorded at for-weeks interval during treatment. In addition, Qufengtang, granular form, was taken twice a day,once each before breakfast and dinner.Results1.After treatment, urinary sediment red blood cell counts were significantly reduced compared with that before treatment there was a significant difference (P<0.001). The mean RBC in the urine after administration were lower than that before. But the result between after treatment 15 days and that of 2 months had no difference.2.immunology index IgA levels before and after treatment are also statistically significant(P<0.05).3. The mean sums of clinical scores before treatment, during the first-month and second-month intervals are 12.67 ± 4.14,6.67 ± 3.03 and 4.80 ± 2.60 respectively. The second-month results of the above measurements are all statistically significant as compared to before treatment(P<0.05)4. According to the curative standard of urine red blood cell count:0 cases of clinical remission,17 cases with significant improvements,8 cases with mild improvements,5 cases with no improvements, significant efficiency was 56.7%, the effective rate was 26.7%, the total effective rate was 83.3%.ConclusionsQufengtang is proven effective in decreasing urinary sediment red blood cell counts. Meanwhile it can also improve clinical symptoms and reduce serum IgA levels, which may imply that Qufengtang may has immunomodulatory effects. |
