Objective: Psoriasis is a common immune-mediated chronicinflammatory disease. The skin pathology is caused by infiltration ofKeratinocytes, T lymphocytes, dendritic cells, macrophages and neutrophilsgranulocytes. Although the mechanism of psoriasis is still not very clear,strong evidences have shown that helper CD4+T lymphocytes play importantrole in the development of psoriasis. Coimbra[1]et al. argued that Th17cellsparticipated in the onset of psoriasis and had close relation with thedevelopment of the disease. Besides, lots of researches reported that Th17cells count in peripheral blood increased. The process of the onset of psoriasiscould be shortened by inhibiting the formation of Th17cells. Recent studiesfind that cytokine IL-21could enhance CD4+T cell’s transformation intoTh17, which could indirectly affect the pathogenic process of psoriasis. Byusing mice models, Qing[2]et al.found that IL-31could enhance thetranscription of IFN-γ in Th1cells, which could significantly inhibit theformation of IL-17A/F mRNA in Th17cells. The experiment by doubleantibody sandwich ELISA for quantitative detection of IL-21and IL-31expression in the serum of patients with psoriasis vulgaris. To assess therelationship between the clinical lesion area and severity (PASI) score.Explore the relationship between IL-31and IL-21pathogenesis of psoriasisand the development of the disease between.We detect the expression level ofinterleukin-21(IL-21) and interleukin-31(IL-31) in blood serum of psoriasisvulgaris, compared with healthy control volunteers, In order to investigate thecorrelation the expression of IL-21and IL-31with the incidence of psoriasisvulgaris, further discussing the Th17cells and the effect factor IL-31, IL-21inthe psoriasis pathogenesis role.Methods:Patients with psoriasis vulgaris from the dermatology clinic of the Second Hospital of Hebei Medical University during March2011toDecember2012were choosed as the case group. Selection standard: P atientswith psoriasis vulgaris had typical skin lesions and a clear diagnosis, had notreceived system treatment of glucocorticoids, immunosuppressants and otherdrugs which affected the immune function in last January before theexperiment, and none of them had autoimmune diseases or other seriouschronic systemic diseases, such as systemic lupus erythematosus, diabetes.Normal control group came from healthy hospital care staff. The case grouphad23cases,12males and11females, age range19to53years, mean age33.30±11.30years. According to the disease stage, case group were dividedinto the progressive stage group of12cases and stationary stage group of11cases. PASI score was evaluated by the same one according to psoriasis areaand severity. Normal control group had11cases,5males and6females, agerange20to52years, mean age27.27±8.63years. Through t-test, the sex andage had no significant difference with the case group. Adopted venous blood2ml from all subjects into EDTA anticoagulant tubes, centrifuged to collectserum samples and saved at-20°C refrigerator. Finnaly detected expression ofIL-21and IL-31in the surem by double antibody sandwich ELISA test.Applicated SPSS13.0software for statistical analysis, datas of case group andcontrol group, progressive stage group and stationary stage group showednormal distribution by normality test, so the statistical results were describedby mean±standard deviation, we used two independent sample t test toanalyze whether the difference between case group and control group,progressive stage group and stationary stage group about the expression of IL-21and IL-31in the serum had significant difference or not. The psoriasisPASI score value data were skewness distribution by normality test. We usedthe variables rank correlation analysis (Spearman method) to analyze therelationship between the expression of IL-21and IL-31in the surem of casegroup and PASI score values repectively. P <0.05had statistically significance.Result:1The expression level of IL-21in serum of case group104.68±42.71was higher than that of normal control group80.03±32.44, the difference was statistically significant (t=-2.500P=0.018P<0.05). Progressive stage groupwas114.95±31.42and stationary stage group was93.49±24.54, the differencewas statistically significant (t=3.304P=0.003P<0.05). The expression levelof IL-21in the serum of case group and PASI score value had positivecorrelation (r=0.448P=0.032P<0.05).2The expression level of IL-31in serum of case group231.30±104.97was higher than that of normal control group160.40±53.34, the difference wasstatistically significant (t=-2.679P=0.012P<0.05). Progressive stage groupwas277.25±98.59and stationary stage group was181.18±79.11, thedifference was statistically significant (t=2.149P=0.043P<0.05). Theexpression level of IL-31in the serum of case group and PASI score value hadpositive correlation (r=0.494P=0.017P<0.05)Conclusion:1The expression level of IL-21in the serum of psoriasis vulgaris groupwas higher than that in the normal control group, explained that IL-21participated in the process of psoriasis vulgaris, confirmed the expression ofIL-21has correlation with the pathogenesis of psoriasis vulgaris closely, Inpsoriasis vulgaris serum, the expression of IL-21in progression stage groupwas higher than that in the stationary stage group. confirmed the expression ofIL-21may be increased in the inflammatory response severely and reduced inthe inflammatory response reduced. Its was positively correlated with thedegree of inflammation in psoriasis.2The expression level of IL-31in the serum of psoriasis vulgaris groupwas higher than that in the normal control group, explained that IL-31participated in the process of psoriasis vulgaris, confirmed the expression ofIL-31has correlation with the pathogenesis of psoriasis vulgaris closely, Inpsoriasis vulgaris serum, the expression of IL-31in progression stage groupwas higher than that in the stationary stage group. confirmed the expression ofIL-31may be increased in the inflammatory response severely and reduced inthe inflammatory response reduced. Its was positively correlated with thedegree of inflammation in psoriasis. 3The expression level of IL-21and IL-31in the blood serum of psoriasisvulgaris had correlation with the psoriasis lesions aera and clinical severitydegree PASI score, so IL-21and IL-31can be used as the evaluation severityindex of the disease.
