| Objective:Psoriasis is a common, recurrent, chornic, inflammatory and proliferative skin disease, The clinical mainly is different sizes, inflammatory and red papula or patches distribut in the scalp, trunk and limbs out side, which covered by multilayer, dry, silvery and white squames. The lesions have distinctive film phenomenon and bleeding point apperment. Most patients lesions performance for heavyer in winter, while in summer become light, High recurrence rate seriously impact the patients' physical and mental health and the quality of life, The fundamental pathological performance mainly is characterized by epidermis keratinocytes hyperkeratosis with parakeratosis. Psoriasis may present vulgaris, pustulosa, arthropathica and erythrodermic in clinical. Psoriasis vulgaris is most comme type. Psoriasis vulgaris is divided into progressive stage, stationary stage and degenerative stage by the course of disease. the pathogenesis is still not completely understood. Past research showed psoriasis is a T lymphocyte abnormality, Th1 cells with excessive activation for advantage leads to the autoimmune diseases. Recently, in autoimmune disease and allergens specificity reaction study people found a special produce interleukin 17 (interleukin 17, IL - 17) help type T cells, called Th17 cells, these cells except can specificily secrete IL - 17 still secrete IL - 22, IL -21,etc .when theTh17 cells found, people began to discovery to understand and evaluate the regulatory role of Th1 cells in autoimmune diseases, more about the study of Th17 cells and its effect factors in autoimmune diseases become a focus.We detect the expression level of interleukin-17 (IL-17) and interleukin- 22(IL-22) in blood serum of psoriasis vulgaris, compared with healthy control volunteers, In order to investigate the correlation the expression of IL -17 and IL-22 with the incidence of psoriasis vulgaris, further discussing the Th17 cells and the effect factor IL-17, IL-22 in the psoriasis pathogenesis role.Methods:The case group involved 29 typical vulgaris psoriasis patients who were choosed from June 2010 to November 2010 in dermatologist out-patient service of the second hospital of Hebei Medical University. Select standard: 1 The case group had typical clinical manifestations of psoriasis vulgaris. 2 All the cases involved had not used contains glucocorticoid topical drugs for two week, or taken systemic drug therapy (especially use the corticoateroid hormone or immunosuppressive agent) for one month. 3 Excluded patient with autoimmune disease such as pemphigus, multiple dermatomyositis, systemic lupus erythematosus, rheumatic arthritis, et al, and patients with other severe systemic diseases. the case group consist of 18 males and 11 emales, age range 19 to 52 years old, average age was 31.60±9.29 years old, we divided all the cases to two groups,one group was progressive stage which had 21examples,the other group was stationary stage which had 8 examples. PASI score which was based on clinical manifestations was assessed by same staff . 10 cases who were from our health medical staff was normal control group. the group consisted of 5 males and 6 emales, age range 25 to 40 years old, average age was 29.24±12.11 years old ,Through t-test ,sex and age had no significant differences with the case group. Adopted vulgaris psoriasis patients elbow vein blood 2ml for anticoagulation tube with 10% EDTA (1mg/ml) and aprotinin (500kIU/ml), centrifugated samples with 3000 revolutions per minute for 20 minutes. Then the serum samples were frozen at -70℃in the refrigerator of preservation. Application of double antibody sandwich-type enzyme-linked immunosorbent assay (ELISA) detected the expression level of IL-17 and IL-22 in blood serum,The statistical data was carried out by SPSS13.0 software, The statistical data showed skewness distribution by the test of normality, so we used two independent sample Wilcoxon signed-rank test, results describedwith median (quartile rang), for the expressed level of IL-17 and IL-22 in serum, we analyzed betweent the case group and the normal control group, and the progressive stage group and the stationary stage group, p<0.05 had statistical significance.We also analyzed the relation between the expressed level of IL-17 and IL-22 in serum and the clinical lesions area and severity degree (PASI score) by spearman rank, p<0.05 had statistical significance .Result: The result of enzyme-linked immunosorbent assay was showed, the expression level of IL-17 in psoriasis vulgaris serum was higher than that in the normal control group (u=-1.696 P=0.045 P<0.05), the difference was statistically significant, the progression stage group was higher than the stationary stage group (u=-2.074 P=0.019 P<0.05), the difference was statistically significant, the expression level of IL-17 and the clinical area and severity degree (PASI score) had not linear correlation (r=-0.048 P=0.799 P>0.05); the expression level of IL-22 in psoriasis vulgaris serum was higher than that in the normal control group (u=-1.727 P=0.042 P<0.05), the difference was statistically significant, but the stationary stage group was higher than the progression stage group (u=-0.049 P=0.480 P>0.05), the difference was not statistically significant, the expression level of IL-22 and the clinical area and severity degree (PASI score) had not linear correlation (r=-0.149 P=0.442 P>0.05).Conclusion:1 The expression level of IL-17 and IL-22 in the serum of psoriasis vulgaris group was higher than that in the normal control group, confirmed the expression of IL-17 and IL-22 has correlation with the pathogenesis of psoriasis vulgaris closely, and indirectly reflected the Th17 cell in the pathogenesis of psoriasis had an important role.2 In psoriasis vulgaris serum, the expression of IL-17 in progression stage group was higher than that in the stationary stage group, indicated IL-17 play an importan role in flammatory progresses of psoriasis lesions, might be concerned with it as former inflammation mesons mainly play an important function in the progress of inflammation.3 The expression level of IL-17 and IL-22 in the blood serum of psoriasis vulgaris had not correlation with the psoriasis lesions aera and clinical severity degree PASI score, explained no specific relevance with the expression level of IL-17 and IL-22 and the area of psoriasis lesions and severity, so IL- 17 and IL-22 were not reflected the severity index of the disease. |
