ObjectivePsoriasis is a chronic inflammatory disease.Its pathogenesis partly often overlaps with other immune-mediated inflammatory diseases.So it usually combines cardiovascular disease,metabolic syndrome,inflammatory bowel disease and so on,in which the occurrence of cardiovascular events can be observed especially.Currently,there are no general laboratory indicators for assessing the level of psoriasis activity.The neutrophil to lymphocyte ratio is an emerging load index of systemic inflammation,which can often be used to monitor the progression of chronic inflammatory diseases and predict the occurrence of cardiovascular events.This study was aimed to observe the differences in NLR and other relevant parameters between patients with chronic psoriasis and healthy controls,except for the risk factors of traditional atherosclerosis.Besides,we evaluated the correlation between the NLR and the quantitative index of the severity of psoriasis vulgaris-PASI score,which may provide new ideas to monitor psoriasis activity in clinical diagnosis and predict its associated CVD complications for dermatologists.MethodThis study included 102 patients with chronic psoriasis who were treated in the Department of Dermatology and Sexually Transmitted Diseases of our hospital from June 2018 to December 2018 as an observation group,and healthy people who were enrolled in our hospital at the same time and matched in numbers,genders and ages as a control group.We created a relevant excel form for recording the general baseline characteristics of the subjects,various hemodynamic parameters,relevant blood cell parameters,biochemical indicators and calculating PASI score,FRS score,NLR,PLR,BMI,MAP,PP values.We analyzed the differences in the neutrophil absolute values,NLR,PLR and other inflammatory parameters as well as metabolic parameters such as TG,LDL and SUA between the two groups of subjects.Besides,we detected the association between PASI scores and NLR in the case group.Result1.General baseline data:The average ages of the case observation group was 41.06±12.33 years old,and the male proportion was 58.8%.There was no statistical difference between the two groups?P>0.05?.The BMI and FRS scores in the case group were higher than those in the control group,which was statistically significant?P<0.05?.In the case group,the positive family history was accounted for 21.6%,the average disease duration was 13.31±12.13 years,the plaque-type psoriasis was accounted for 85.3%in the case group,and the mean PASI score was 16.63±8.91.2.Comparison of hemodynamic parameters:The average heart rate of the case group was81.14±8.00 beats/min,which was higher than the healthy control group.And the difference was statistically significant?P<0.05?.While other hemodynamic parameters of the case group were compared with the control group,it was no statistical difference?P>0.05?.3.Comparison of blood cell parameters and biochemical datas:We found WBC?6.60±1.58?,neutrophil count?3.81±1.18?,NLR value?2.22±0.99?,PLR value?139.21±44.80?,TG?1.65±1.82?,LDL?3.12±0.73?,SUA?346.68±90.63?in case observation group were higher than the control group,and HGB?138.76±14.51?was lower than the control group,which was statistically significant?P<0.05?.There were no statistical differences in other blood cell parameters or biochemical parameters compared with the control group?P>0.05?.4.Comparison of the NLR in patients according to PASI scores and correlation analysis of the two variants:The NLR values of PASI>20 group were significantly higher than those of PASI10-20 group and PASI<10 group,the difference was statistically significant?P<0.05?.However,there was no significant difference in NLR values between the later two groups.NLR was positively correlated with PASI score(rNLR=0.456,PNLR<0.001).Multiple linear regression models showed that NLR was an independent predictor of PASI score?B=3.240,P=0.001,95%CI:1.256-4.848?.5.Diagnostic efficacy of NLR for high PASI score?PASI>20?psoriasis:the area under curve?AUC?of identifying high PASI score psoriasis was 0.644,the optimal cut-off point was 2.686,95%CI was 0.612-0.827,the sensitivity was 64.5%,the specificity was78.3%,and the P value was<0.001.ConclusionThe NLR value of patients with chronic psoriasis vulgarisis was higher than healthy people,and it was positively correlated with PASI scores,indicating that patients with psoriasis may be in the systemic subclinical inflammatory conditions.NLR could be used to monitor the degree of disease activity and track the systemic inflammatory conditions,which may reduce the risk of early cardiovascular diseas.
