Public hospital is the main body of our country’s medical system. It is the public welfare institution for the protection of people’s basic medical service demand by government financial investment. Its development conditions should be to a considerable extent which can synthetically reflect our national quality of life and social welfare. Public hospitals to solve our country current serious " the illness " undertakes the important responsibility of "public welfare ". According to our country in three different periods of "health care" system reformation, its management system is crucial to solve " the illness", which also be important to proceed with the reform of the public hospitals. Since "the new medical reform" in2009, the goverment has issued a series of public hospital management system reform policis and guidances, which has basically established the four separate reform target of the "separate government administration from medical institutions, management from operation, medical care from pharmaceuticals, and for-profit from nonprofit operations", and now which explores the establishment of the council and other forms of public hospital corporate governance structure. From the present situation of the reform, some areas and public hospitals have formed a unique mode of the management.This research based on the current " four separate" reform guiding target and "public hospital reform pilot background ", selected five representative public hospital management system reform practice mode, which are "Weifang hospital management autonomous pattern "," Shanghai Shenkang hospital management institutional mode "," Jiangsu Suqian hospital privatization model"," Wuxi hospital trusteeship management model " and " Beijing Haidian hospital contract management mode ". And this research introduces the Preker-Harding model to carry on the comparative analysis, and tries to put forward a reform and optimization of mode, with period the public hospital reform provides research proposal. This is both in the macro for national guiding the commonweal welfare track of public hospital, solving the problem of the " illness problems ", or in the micro for realization of public hospital scientific management, enhancing the public hospital corporate management efficiency,which has important research meaning.This research includes six aspects:The first chapter is the introduction part, which gives the research background and the significance, domestic and foreign scholars research results, research methods, research thought and innovation; The second chapter is the related concept and theory analysis, which defines the public hospitals and public hospital management system, and introduces the reform value of the " people-oriented, fair, public welfare ", and provides a brief analysis of the "new public management theory, the separation of the two rights theory and the principal-agent theory"; The third chapter is the reform situation analysis of our country’s public hospital management system, which introduces the public hospital management system reform process in three different periods, elaborates four contradictions in the reform of the public hospital management system, and analyses the possibility factors of the impact of public hospital management system reform; The fourth chapter introduces five reform practice models of our country’s public hospital management system; The fifth chapter is the analysis based on the Preker-Harding model of public hospital management system, which uses this model mechanism to discuss our country’s public hospital management system from five dimensions, and analysises the evaluation of five reform models from eight aspects, and finally puts forward some thinking about the compare of these reform practice modes; The sixth chapter is the conclusions and suggestions.This thesis draws four conclusions:Public hospital management system reform needs government which must clear its position in the public hospital reform and determine the appropriate financial compensation responsibilities for public hospitals;"Public hospital pilot" and "reform environment" is not coordinated;The "four separate" reform guiding principle can effectively guide public hospital to regress the orbit of public welfare;The "four separate" do not speak system problem, but management scientific problems.This thesis has two innovation points:one is the background of innovation,which based on the "Medical Reform" and "Public hospital pilot";the two is the analysis tool of innovation,which use Preker-Harding model to undertake an analysis. Of course, more researches are needed in future since this paper is a groping and defective one.
