Preliminary Research On Establishing Evaluation Indicator System For Social Responsibility Of Tertiary Public Hospitals

[Objective]This research is to use qualitative and quantitative methods to develop an comprehensive and scientific evaluation indicator system for social responsibility of public hospital with high validity and reliability for evaluating tertiary public hospital, which is suitable for the current development situation of tertiary public hospital, and easy to carry out and spread, and thereby provide reference and basis for strengthening scientific, standardized and normalized administration of health services of tertiary public hospital for health administrative departments and organizations.[Methods](1)The literature and information analysis, personal interview method, focus group method, field survey, Delphi method was applied to establish the indicator system for social responsibility of public hospital.(2)Searching and analyzing the articles about the basic theory, method and related research results of the company society responsibility and social responsibility of public hospital, to find the connotation and extension of social responsibility for tertiary public hospitals and its evaluation indicator system.(3)The experts interview, expert consultation was used to screen the evaluation indicators of social responsibility evaluation for tertiary public hospitals, preliminary establish evaluation indicator system.(4)Formatting the patient satisfaction questionnaire, and calculating the general Cronbach coefficient and clausal Cronbach coefficient, to detect the internal consistency of the questionnaire. Spearman-Brown related analysis was used to test and evaluate the splithalf reliability and test-retest reliability of the questionnaire for patients. The correlation analysis and factor analysis method was used to test the content validity and structural validity of the questionnaire.(5)Logistics regression analysis was used to evaluate the influencing factors of patients for tertiary public hospitals.(6)RSR method and weighted Topsis method was used to comprehensively evaluate the indicator system for tertiary public hospitals.[Results](1)The evaluation indicator system for tertiary public hospitals was set up, which included4dimensions and28indicators. The fairness and accessibility dimension contains10indicators, including "proportion of person participating in public health emergencies emergency". The Suitable dimension contains9indicators, including "cross subsidy index". The quality dimension contains4indicators, including "score of quality management years". The efficiency dimension contains5indicators, including "Outpatient(Emergency)services per year per person"(2) This study made a patient satisfaction questionnaire including three primary indicators and12secondary indicators. In this questionnaire, general Cronbach coefficient was0.906, and the standardized Cronbach coefficient was0.907, which could explain higher internal consistency reliability of the questionnaire. The Cronbach coefficient of service quality was0.856, service suitability was0.830, and professional morality was0.825. There are12secondary indicators for the corresponding12problem; delete the highest indicator, the Cronbach coefficient was0.789to0.878, which showed that questionnaire reliability was good.(3)The correlation coefficient of "red envelopes, rebate" was0.399, which was lower than0.4. The correlation coefficient value of the rest correlations were between0.4and0.6(P<0.05). Factor analysis was used to evaluate structural validity, in addition to "reject red envelope"; the other main factors of each option could map the Class one indicators which designed at the beginning. This indicated that the item of the questionnaire was divided reasonably, and could effectively reflect social responsibility evaluation for patients, the structure of the questionnaire validity was good.(4)According to the Logistic regression analysis results, from the point of view of the patients, the social responsibility of tertiary public hospital was related to hospital environment, the doctor diagnosis and treatment prescription, reject to red envelopes, equal treatment for different insurance forms of patients, and treatment first and balance latter. These factors effectively showed expectations of the patients to tertiary public hospitals for as a representative of the public medical institutions.(5)The headscarf Kendall’s W collaborative coefficient test RSR method and Topsis method was used to evaluate consistency of social responsibility evaluation. Kendall’s W collaborative coefficient was0.745, significant degree for Asymp.Sig was0.145, which could not say the results of the two methods had the obvious consistency. Further evaluation methods should be made in future researches.[Conclusion](1) The evaluation indicator system has the reasonable structure, evaluation the completeness of the contents.(2) Using Delphi method of expert consultation evaluation indicator selection, result is more reliable.(3) Patient satisfaction questionnaire design has high reliability and validity.(4) The gain rate of some evaluation indicators in questionnaire for tertiary public hospital is not very well, further research should be made.
