Correlational Study Of Adenoid Morphology And Secretory Otitis Media In Children

Objective:To discuss the correlation of adenoid morphology and SOM in children by compared with the children’s adenoid general form, imaging form, cytology form three aspects and the diagnosis and treatment of pediatric SOM.Methods:By case observe study,98children cases or184ears(double ear suffered86cases, single ear suffered12cases) who have suffered SOM and had adenoidectomy in2011.01-2011.10are made related inspections in otolaryngology head and neck surgery of the third Affiliated Hospital of ZunYi Medical College. The inspections are included of three-dimensional CT of nasopharynx portion, fibroptic nasopharyngoscope, acoustic immittance measurement, pathologic inspection and ABR in necessary to rule out of cochlear and retrocochlear lesions. The patients who are older than5years are made pure-tone hearing threshold testing and in the operation auripuncture for B tympanoplasty are made.98cases, who have been diagnosed as children SOM except carcinoma of nasopharynx, meningoencephalocel, middle ear of the trauma, are taken as the research object. Divided them into three groups:SOM caused by only hypertrophy adenoids,SOM caused by inflamed adenoids and SOM caused by allergic adenoids.:Combined with related literature in recent years, the correlation of adenoid morphology and SOM in children are evaluated and analysed by the children’s adenoid general form, imaging form, cytology form.Results:In98cases, SOM patients caused by adenoids lesions who are comfirmed by auripuncture26cases (26.5%),patients of hearing loss who are comfirmed by Acoustic Immittance Measurement and Pure Tone Test72cases (73.5%).It is found that the clear correlation of adenoid morphology and SOM in children by the children’s adenoid general form, imaging form, cytology form.Especially, adenoid morphology is connected with pharyngeal opening of auditory tube.In the inspection of adenoid morphology,it is easy to catch SOM for the kind of hypertrophy, A/N≥0.71, inflamed adenoids and allergic adenoids.These factors are easy to lead to eustachian tube dysfunction that make tympanogram abnormal and cause to lose hearing of children.On the contrary,the kind of the flat, A/N≤0.60,only patients of adenoidal hypertrophy have less influence on children’s hearing.Conclusion:Adenoid morphology have relation with children’s SOM,so it is necessary to make routine checks which include hearing inspections, three-dimensional CT of nasopharynx portion, adenoid tissue for cytologic examination.As is further help to aim at diagnosing or ruling out of SOM.
