AimThis study aimed at investigating the etiology, pathological characteristics,management and prognosis of chronic pancreatitis in the Chinese population.142patients with chronic pancreatitis were recruited in Union Hospital betweenJanuary2008and December2011. The age ranged from14to76years (average43). The etiology, pathology, management and prognosis of these patients wereretrospectively analyzed.ResultsThe etiology data showed that most of the patients were obstructive chronicpancreatitis,72(50.70%), while the others were alcoholic CP,20(14.08%),autoimmune CP14(9.86%) and idiopathic CP36(25.35%). The average diameterof the inflammatory mass was3.8±3.3cm (mass range2.8cm to4.8cm). CA19-9was significantly increased in27patients (during intraoperative frozen section7patients were found to have pancreatic cancer). Ninety four patients weremanaged surgically,34patients by endoscopic sphincterotomy and14patientsconservatively. The duodenum preserving total pancreatic head resection withoutresection of segment of duodenum was performed in42patients. Berg’sprocedure was performed in25patients; Whipple’s were performed in7patients,cystojejunostomy in18patients and caudal pancreaticojejunostomy in2patients.No patient dead after operation. Operational complications of pancreatic fistula, abdominal bleeding, wound dehiscence, bile leakage and duodenum fistulaoccurred in3cases,4cases,2cases,2and1case, respectively. Pain relief wasachieved in76patients. Recurrence of pain was seen in17patients. Postoperative weight gain was observed in all94patients.ConclusionPancreatic duct obstruction remains the most common cause for CP in Chinesepopulation. The duodenum preserving total pancreatic head resection isappropriate treatment for CP complicated with inflammatory mass. The increasein CA19-9is a high risk for chronic pancreatitis to become pancreatic cancer.
