Objective Severe acute pancreatitis(SAP) occurred in 15% ~20% of patients with acute pancreatitis, which was morphologically characterized by extra and intrapancreatic necrosis and associated with single or multiple organ failure syndrome(MOFS).It was generally accepted that surgery can be considered of major form of infected pancreatic necrosis. However, timing and indication of SAP have been still controversial. In this retrospective study, the effects of different kinds of treatment were evaluated, and the relationship between timing of laparotomy and prognosis were analyzed. Methods A total of 79 consecutive SAP patients were admitted to our hospital from January 1999 to January 2004. All patients were diagnosed by Computed Tomography and clinical testing parameter or laparotomy .Timing of laparotomy, surgical complications and mortality rate were discussed. Results Complication rates in group of early,delayed laparotomy and conservative treatment were 33.33%,11.11%,7.14% respectively. Their mortality rate were 15.15%,5.56%,3.57% respectively . Conclusion Strategy in management of SAP was an important factor influencing the prognosis of SAP. Individual treatment (including delayed surgery and conservative form) in SAP may have good prognosis (with low complication and mortality rate).
