Objectives:To evaluate the reliability and validity of Chinese Dementia Quality of Life Scale (DQoL); learn about the modern status of dementia patient’s quality of life and influencing factors in China and Japan. Then we make a comparison and explore the reasons for differences in order to provide theoretical bases for the future nursing intervention, and improve the patient’s quality of life.Methods:We translated the DQoL into Chinese, and produced a5-point scale which comprised of3test questions,29items and1overall QoL item. Then, according to MMSE, we chose43patients with mild-to-moderate dementia from Jinan city to evaluate DQoL’s reliability and validity.Then a total of200mild-to-moderate dementia patients in hospitals, apartments for the elderly in Jinan city and a total of205mild-to-moderate dementia patients in nursing homes, care houses, group homes in Wakayama city were recruited by convenience sampling method. We used Demographic Information Questionnaire, Mini-mental State Examination (MMSE), Barthel Index (BI), Dementia Quality of Life (DQoL) and Neuropsychiatric Inventory Questionnaire (NPI-Q) to learn about the status of patient’s quality of life and analyze the relation between general demographic information, activity of life, behavioral and psychological symptoms and quality of life. Before investigation we gave the interviewers a unified training, during the investigation, the interviewer delivered the questionnaires one by one and recovered them on the spot. In Japan, we used the same method. SPSS16.0was used to perform statistical analyses. We measured the reliability and validity of DQoL with Cronbach’s a, Pearson correlation analysis and content validity index; explored the influencing factors of dementia patient’s quality of life with t-test, one-way anova, pearson correlation analysis and multiple stepwise regression analysis. Results:1. The reliability and validity of Chinese DQoLThe internal consistency reliability Cronbach’s a coefficient of Chinese DQoL was0.856; the test-retest correlation coefficient was0.726; the content validity index for scales was0.986; the correlation coefficient among the five dimensions ranged from-0.133to0.694.2. The score of Dementia Patient’s Quality of LifeThe total score of DQoL in Jinan city was89.82±15.11, the scores of five dimensions were, self-esteem:10.79±3.28, positive attitude:16.82±4.47, negative attitude:42.12±7.87, belongings:6.99±2.78, aesthetics:13.12±4.20; The total score of DQoL in Wakayama city was118.30±14.56; the scores of five dimensions were, self-esteem:15.86±3.17, positive attitude:21.92±4.89, negative attitude:48.79±6.67, belongings:10.73±2.58, aesthetics:21.00±3.44. multiple linear regression showed that the influencing factors of quality of life were education, BMI, ADL and depression.As for the overall rating on quality of life, in Jinan city, most of the patients(44%) thought their quality of life were good,38%patients thought them fair, the ratio of "very good","bad" and "excellent good" were12%,5.5%and0.5%respectively; in wakayama city, most of the patients(35.6%) thought their quality of life were fair,29.3%patients thought them good, the ratio of "very good","excellent good" and "bad" were24.9%,9.8%and0.5%respectively. Then we collapsed the five grades into two parts,"bad" and "fair" was1,"good","very good" and "excellent" was2, and used the x2test, the result showed that there was no statistically significant difference between the two cities.3. The influencing factors of Dementia Patient’s Quality of LifeThe single factor analysis showed that there were significant differences among different age, ADL group, characteristics, economics, marital, payment, hypertension and neuropsychiatric symptoms(depression, irritability) in Chinese patients; there were significant differences among different gender, ADL group, religion, education, BMI group, characteristics and neuropsychiatric symptoms(depression, elation) in Japanese patients.Multiple stepwise regression analysis showed that the influencing factors of quality of life in Chinese patients were marital, economics, characteristics, ADL, hypertension and irritability; the factors in Japanese patients were education, BMI group, ADL and depression.Conclusions:1. The Chinese version of the Dementia Quality of Life has good reliability and validity, however the sample of this investigation is a little less, the scope is limited and deserves to do further research in a larger sample.2. In this study, the total score of DQoL in Jinan city is significantly lower than Wakayama city; as for the overall rating on QoL, there is no significant difference.3. The same influencing factor between these two cities was ADL. In addition, marital, economics, characteristics, hypertension and irritability are also the influencing factors in Jinan city; education, BMI group and depression are also the influencing factors in Wakayama city.
