Investigation And Research On Current Situation And Countermeasures On Diabetes Health Management In Community In Yuhang District Hangzhou City From The Perspective Of HOPE Model

Objectives:To promote health, diabetes health management in communities was studied as the starting point, through research on the current situation of sample community, advantages and disadvantages on the diabetes health management were summarized, reasons for the disadvantages were analyzed, countermeasures were proposed to improve diabetes health management in communities.Methods:Understood the background knowledge of diabetes, health management theory and research status of diabetes health management at home and abroad through a review of literature; did a descriptive analysis on the current situation of diabetes health management in community from a perspective of HOPE model by looking up relevant information and integrating the methods of observation and interview; summarized the advantages and disadvantages; analyzed the reasons of the disadvantages; proposed countermeasures on improving diabetes health management in communities.Results:Advantages:health supportive environments was created through health promotion(P), the diabetes detection rate, standardized management rate and the glucose control rate were of good results, he satisfaction of diabetes patients and superior management departments was of high level. Disadvantages:the health management plan was lack of health-centered(H) goal decomposition and quantization index, job responsibilities and the specific work index, schedule and specific action content; the organization(O) of the plan was short of health resources such as medical personnel, devices and service funds, the health evaluation and interventions was lack of instructive theory and methods; the results of the implementation of the health management plan was lack of all-sided evaluation(E).Reasons:the uneven allocation of health resources, health management in community was of weak foundation, the service capabilities were weak, the personnel training mechanism on health management was not well organized. Countermeasures:strengthen the protection on health management by establishing laws and policies, speed the personnel training of health management, regulate the diabetes health management process, improve the utilization rate of electronic health records, introduce some mature health assessment technology and tools.Conclusions:Studying the current situation of diabetes health management in community from the perspective of HOPE model (Health, Organization, Promotion, Evaluation) is beneficial for finding out the disadvantages on the managerial work,advancing countermeasures, and of practical significance for promoting health.
