Research background:OSCE, objective structured clinical examination, is applied for impersonally evaluating clinical comprehensive ability of clinical undergraduate. In recent years, some inland nursing colleges have adopted this new medical education evaluating mode to estimate students’ comprehensive ability and have made some progress. However, the mode of OSCE applied by most nursing colleges referred to that of clinical medicine from examination station setup, examination procedure and standard clinical records, which could not entirely features nursing subject and could not impersonally evaluate clinical comprehensive ability of nursing undergraduate. Meanwhile, in the whole process of OSCE for nursing undergraduate, both excogitation of standard clinical records and examination method lay particular emphasis on clinical diagnosis, which could not entirely and impersonally evaluate clinical comprehensive ability of nursing undergraduate. At present, no relevant research is reported at home and abroad on standard clinical records in OSCE for nursing undergraduate. So, how to improve OSCE mode of nursing, especially to explore standard clinical records being suitable for nursing, will be our main work direction. We expect OSCE of nursing undergraduate feature nursing subject so that it could objectively and scientifically evaluate clinical comprehensive ability of nursing undergraduate.Research purposeThe study concerns the request of clinical comprehensive ability examination in OSCE for nursing undergraduate. The standard clinical record will be designed on practical clinical circumstances so that the clinical comprehensive ability of nursing undergraduate might be impersonally evaluated. It is expected that nursing undergraduate may cultivate and improve their clinical comprehensive ability through nursing standardized clinical record examination to help them during their nursing practice.Research method1. In accordance with Global Minimum Essential Requirements for undergraduate medical education and Teaching Program of Shanxi Medical University, the basic design and examination request of nursing standardized cases are preliminary determined and relevant experts on nursing education, nursing management, clinical nursing will be consulted by method of Delphi.2. Affairs on examination content, pattern and evaluating standard adopted in OSCE station for nursing undergraduate will be determined by extensively consulting clinical nursing experts by method of interview.3. On the basis of nursing standardized cases, the examination pattern and content of OSCE station will be designed.208nursing undergraduate will be tested in pre-examination. The test result will be analyzed by statistics from difficulty, discriminate validity, credibility, validity and so on. Then concrete examination project of OSCE station for nursing undergraduate in June2011will be ultimately determined.4. In june2011,794nursing undergraduate of different college course and different degree will take OSCE on clinical comprehensive ability. The results will be analyzed by statistic so that undergraduate’s clinical comprehensive ability might be objectively analyzed and evaluated.5. SPSS17.0software is applied in statistic analyze:(1) the test result will be statistically described;(2) examination questions will be analyzed by difficulty and discriminate validity;(3) test credibility and validity such as a value of Cronbach coefficient, criterion validity related validity will be analyzed and calculated;(4) Hotelling’s T-test and LSD-T test of score difference in different course and degree will be calculated.6. By questionnaire survey and having informal discussion in teachers and clinical experts participating in the examination, the opinions on design of OSCE nursing standardized cases, testing pattern, testing quality and efficiency are collected to further analyze and summarize examination results and design experience of nursing standardized cases, providing reference proof for improving nursing OSCE.Results:1. In accordance with Global Minimum Essential Request for undergraduate medical education and Teaching Program of Shanxi Medical University, by resulting and interviewing clinical nursing experts, under professional guidance of teachers in relevant teaching and research sections, the design of nursing standardized cases is ultimately fulfilled.2. By organically integrating nursing standardized cases and nursing undergraduate OSCE, we designed the clinical comprehensive ability examination station. By method of Delphi, we consulted and interviewed the experts and ultimately determined the examining project.3. By preliminary experiment, we analyzed the credibility, validity, difficulty, discriminate validity and so on, and statistically analyzed the testing score by average value, standard deviation, test of normality. It is indicated that, the nursing standardized cases designed in this research are suitable for nursing undergraduate’s OSCE station.4. By testing nursing undergraduate of2011degree, and statistically analyzing their scores, we found that, nursing standardized cased applied in nursing undergraduate’s OSCE can objectively evaluate their clinical comprehensive ability, indicating the actuality and insufficient.Conclusion:In accordance with Global Minimum Essential Request for undergraduate medical education and Teaching Program of Shanxi Medical University, integrating clinical authentic cases, we designed nursing standardized cases suitable for nursing. On basis of design of nursing standardized cases, we prompted reforming present testing pattern and content of nursing undergraduate OSCE station;By analyzing nursing undergraduate OSCE, we found that, the nursing standardized cases designed here are capable to evaluate and examine nursing undergraduate’s ability of communicating, collecting information, managing, observing patient’s condition, critically thinking, clinically determining, and applying professional knowledge into clinic, so that objectively evaluating the current situation of nursing undergraduate’s clinical comprehensive ability and indicating its insufficiency.
