Clinical Study Of Severe Periodontal Disease With Dentition Defect By Conical Telescopic Crown Repaired

Periodontal disease is one of the most common oral diseases which obtain opposite ends for different attitude of patients. It may be improved or quiescent couple with timely integrated medical treatment and actively cooperate with doctor. Conversely, the patient who neglected the dangers of periodontal disease would have to accepted the ends including alveolar bone absorption, loose teeth and dentition defects which named severe periodontal disease.To restore dentition defect caused by severe periodontal disease is one of most complex and difficult problem in the dental clinic although the methods of repair were varied which including removable and fixed prosthesis. For the particularity of SPDDD removable and fixed prosthesis would not arrived at perfect clinical results in long-term. Telescopic is a combined prosthesis between fixed and removable restoration whom have a large number of domestic and foreign studies had taken benign long-term results. In this study, we designed a synthetical assessment system for conical telescopic crown repaired severe periodontal disease which examining relevant indices to evaluated the feasibility of this plan. We hope to provided a theoretical and clinical parameters according to this experiment.Objective:To restore SPDDD by conical telescopic crown and evaluated clinical efficacy synthetically.Methods:12 SPDDD patients were selected and received evaluation about whether periodontal disease improved or quiescent after systemic periodontal therapists. Teeth root canal therapy were produce in adjustment in adjustment abutment and repaired by conical telescopic crown.There were 75 abutments and 15 telescopic crowns been produced. Recording the patient's subjective feeling and particular parameters of periodontal examination to evaluated efficacy at 6th month,12th month,18th month.Results:1,Patients satisfaction rate about appearance, chewing function and comfort of telescopic crown were more than 80%.2,Changes of periodontal indexes:①Probing depth:There were significantly difference between pre-restoration and post-repairation (P<0.05). At the same time the periodontal pocket were lighter and relatively stable, which underwent no significantly changes during the period of post-repairation. (P> 0.05)②The mobility of teeth:The number of movable tooth became lesser and floating tooth had a increasing solid stability during period of post-repairation.③GI:Gingival condition were relatively stable, so there were not significantly changes between pre-restoration and post-repairation (P> 0.05)or during the period of post-repairation (P> 0.05).3,The X-ray of 60 (80%) physically tooth showed alveolar bone became more intensive after telescopic crown repaired and the height of alveolar bone were invariable. The density and height of alveolar bone were constant in 12 (16%) physically tooth. Only 3 (4%) physically tooth were extracted due to severe absorption of alveolar bone.Conclusion:Conical telescopic crown to SPDDD which useful to relieve periodontal disease not only have favorable clinical effects but also playing the role of arch splint to stabled the movable tooth.
