Objective:In order to explore the role of serum 1,25-Dihydroxyvitamin D3 in the development of type 2 diabetes mellitus and diabetic nephropathy.Methods:Levels of scrum 1,25-Dihydroxyvitamin D3,glucose, lipid,HbAlc, creatinine,calcium,phosphorus,BMI,insulin , insulin resistance index, creatinine clearance rate as well as height,weight and blood pressure were measured and calculated in 64 patients with type 2 diabetes mellitus and 20 healthy subjects.Accoding to urinary albumin to creatinine ratio(ACR) and serum creatinine level,the patients with type 2 diabetes mellitus were separated into 4 groups of normoalbuminuria diabetic group(DM n=16), microalbuminuric diabetic group(M-DN,n=16), proteinuria group(P-DN,n=16) and renal failure group(F-DN, n=16).Analyze the relationship between the above parameters and serum 1,25-Dihydroxyvitamin D3 level among all these groups.Results:(1) The patients with type 2 diabetes mellitus had lower serum 1,25-Dihydroxyvitamin D3 levels than healthy subjects and the difference was significant (P<0.05).(2)Serum 1,25-Dihydroxyvitamin D3 levels were gradually decreased among the four groups of type 2 diabetes mellitus and significant differences were observed in pairwise comparison (P<0.05).(3) Serum 1,25-Dihydroxyvitamin D3 levels were negatively correlated with BMI, diastolic pressure, pulse pressure, total cholesterol, triglyceride, serum creatinine and HOMA-IR while positively correlated with creatinine clearance rate (P<0.05)Conclusion:1.There was an insufficiency of 1,25-Dihydroxyvitamin D3 among type 2 diabetics.2.1,25-Dihydroxyvitamin D3 insufficiency may have a relationship with adiposity, hypertension, hypercholesterolemia, hypertriglyceridemia and insulin resistance.3. Type 2 diabetic nephropathy patients were deficient in 1,25- Dihydroxyvitamin D3. Along with the extent of kidney damage mounting up, serum 1,25- Dihydroxy-vitamin D3 levels decreased by degrees.
