Objective: To investigate the relationship between homocysteine level and insulin resistance in patients with diabetic nephropathy.Method: A total of 106 patients with diabetes admitted to the hospital between January 2018 and June 2018 were divided into simple type 2 diabetes group(A group,a total of 39 cases),microal-buminuria group(B group,a total of 32 cases),and macro-albuminuria group(C group,a total of 35 cases)according to the results of urinary albumin excretion rat(UAER).Equilibrium between groups and The difference was not statistically significant.The three groups were collected and analyzed for blood homocysteine(Hcy),triglyceride(TG),total cholesterol(TC),urea nitrogen(BUN),creatinine(CR),glycosylated hemoglobin(HbA1c),fasting plasma glucose(FPG)and general indicators.Result:(1)There was a statistically significant difference in age,SBP,and duration of disease between the groups.(P<0.05).There was no significant difference in the FPG between the three groups(F=0.064,P=0.938),there were statistically significant differences in HbA1 c,TC,TG,BUN,Cr,Cys C,and Hcy(P<0.05).(2)Hcy was positively correlated with age,HbA1 c,and HOMA-IR in group A,and there is a statistical significance defference(P<0.05),Hcy was positively correlated with age,HbA1 c,BUN,Cr,Cys C,TC and HOMA-IR in group B,and there is a statistical significance defference(P<0.05),Hcy was positively correlated with age,BMI,HbA1 c,BUN,Cr,Cys C,TC and HOMA-IR in group C,and there is a statistical significance defference(P<0.05).(3)Hcy level:A group<B group<C group(F=123.331,P<0.001),HOMA-IR level:A group<B group<C group(F=73.405P<0.001).(4)Multivariate linear stepwise Regression analysis showed that HOMA-IR is independent factor of Hcy.Conclusions: With the progress of the course of diabetic nephropathy,the levels of Hcy and HOMA-IR increased,and there was a correlation between Hcy and HOMA-IR.
