Objective 1.To investigate the changes of the serum Apelin level in patients with type 2 diabetes nephropathy(DN);2.To investigate the mechanism causing the changes of serum Apelin level in the diabetes nephropathy patients.Methods Sixty-four patients with type 2 diabetes mellitus (DM) were selected and divided into diabetes nephropathy(DN) and non-diabetes nephropathy subgroups; thirty patients in normal control group.The level of fasting serum Apelin and fasting insulin were measured respectively, insulin sensitivity was assessed by HOMA-IR, Meanwhile, fasting blood glucose(FBG),lipid profile level and HbA1c were measured, waist circumference,blood pressure,height and weight were also measured, and the BMI was calculated.Resultsâ‘ Compare to normal control group , the Apelin concentration was increased in both diabetes nephropathy and non-diabetes nephropathy groups(P<0.01). The serum Apelin level in diabetes nephropathy group was significantly higher than that of non-diabetes nephropathy groups(P<0.05).â‘¡The level of FINS,Ln(HOMA-IR) in diabetes nephropathy were significandy higher than that of non-diabetes nephropathy groups (P<0.05).â‘¢Correlation analysis showed that the serum Apelin was positively correlated to ln(HOMA-IR),FINS,FBG,HbA1c,SBP,DBP,TC,TG,LDL-C,BUN,Cr (P<0.05).â‘£In a multiple step-wise regression analysis, ln(HOMA-IR),TG and DBP were independent factors related to Apelin (r~2=0.229,r~2= 0.254,r~2=0.22,P<0.05) .Conclusions 1.The serum Apelin level in the patients with the type 2 diabetes nephropathy was significantly higher than that of non-diabetes nephropathy.2.The increase of insulin resistance in diabetes nephropathy patients may be the reason of the increase of serum Apelin level.
