Objective: To observe Xiangsha Liujunzi Decoction of multiple organ failure in patients with the clinical efficacy and gastrointestinal dysfunction on serum motilin, IL-6 and critical illness score. Xiangsha Liujunzi Decoction in the treatment of gastrointestinal dysfunction provide objective evidence for gastrointestinal disorders to provide more optimal. Methods:Nanjing University of Traditional Chinese Medicine Hospital of the Third Affiliated Hospital of Nanjing cum multiple organ failure in patients with gastrointestinal dysfunction as the research object, in July 2009-January 2011 of multiple organ failure in patients with gastrointestinal dysfunction included in this study, Western diagnostic criteria for pneumonia:Western medicine should be consistent with the 1997 amendment to the Fry-mods of the diagnostic criteria. (Emergency first edition of the People's Health Press Science) diagnosed as MODS, gastrointestinal dysfunction TCM diagnostic criteria (intestinal stagnation):(refer to Chinese Journal of Critical Care Medicine 2004; 11 (5) 259-263) comply with Chinese and Western medicine diagnostic criteria. Will be included in the 44 patients were randomly divided into Xiangsha Liujunzi decoction group and control group,22 cases in treatment group and control group 22 cases. Two groups herein are conventional western medicine treatment options include:organ support, anti-infective, phlegm, and conventional therapy for primary disease. In addition to the conventional treatment group therapy plus Western Xiangsha Liujunzi serving soup, Xiangsha Liujunzi soup, day 1, day 2;. Through observation and treatment before and after treatment 3,7 days TCM syndromes associated, APACHEā
”score, bowel sounds, and motilin and IL-6 and other indicators of change, evaluate its efficacy and its mechanism.Results:1. Marshall score of the two groups were significant in the first 3 days decreased (P<0.05), and the first decline in more than the previous 7 days was significantly (P<0.05); 2. two integral values of TCM in section 3 and 7 days were significant decreased (P<0.05), but the treatment group scores decreased more significantly than the control group (P<0.05); 3. IL-6 treatment group than the control group from day 3 appears significantly decreased (P<0.05), and the decline in the more obvious on day 7 (P<0.05); 4. motilin values in both groups 3 and 7 days were significant decreased (P<0.05, P<0.01), but the first 7 days of treatment set of values more than the control group decreased significantly (P<0.05).5. PH value of the gastric mucosa in both groups 3 and 7 days were significant decreased (P<0.05), but the value of the 7th day treatment group decreased more significantly than the control group (P<0.05).Conclusion:Treatment of MODS Xiangsha Liujunzi gastrointestinal dysfunction, compared with Western medicine alone, compared to more effectively improve gastrointestinal dysfunction, and promote recovery of gastrointestinal motility, increasing the secretion of gastrointestinal hormones and reduces the systemic inflammatory reaction. Improved tissue oxygen for bacteria and toxins to help prevent further translocation. May block the gastrointestinal translocation of bacteria and toxins, improve gastrointestinal motility and hormone secretion. Xiangsha Liujunzi Decoction in gastrointestinal dysfunction in critically ill patients in the clinical applications of objective evidence; to the treatment of gastrointestinal dysfunction in critically ill patients more than one choice; to provide patients with a more reasonable treatment options, verify that Western Medicine is reasonable. Development of Integrated Traditional and Western medicine, or even multiple organ failure may reduce mortality. |
