Objective:By observing the clinical study on treatment of mild perimenopausal hypertension by claming the liver to suppress Yang and nourishing the liver and kidney, evaluate the clinical efficacy of TCM tretment by claming the liver to suppress Yang and nourishing the liver and kidney.Methods:Devide the 55 perimenopausal women with hypertension who in line with Yin deficiency and hyperactive Yang type in Clinical Department of Cardiology, Jiangsu Provincial Hospital from June 2010 to February 2011 randomly into test group and control group, test group include 28 cases, in which group patients take enalapril with Chinese medicine decoction which can clam the liver to suppress Yang, nourish the liver and kidney meanwhile; the control group 27 cases use enalapril without Chinese medicine decoction.the whole course of treatment is 4 weeks. During the study blood pressure, heart rate, blood lipid, blood glucose, and female hormone and other clinical symptoms before and after the trement are observed, safety of the treatment is also concerned.Results:1. Changes in blood pressure:both the test group and the control group whose blood pressure decreased significantly after treatment, and the difference is significant (P<0.05); compared this two groups after therapy, systolic blood pressure in test group was significantly lower than the control group (P<0.05), but the decrease in diastolic blood pressure of both two groups had no significant difference(P> 0.05). and comparison of therapy indicators of blood pressure between the two groups had no significant difference (P> 0.05).2. Changes in Heart rate:heart rate decreased significantly after treatment in both two groups than before (P<0.05); and the comparison between the two groups after treatment had significant statistical difference (P<0.05), heart rate decreased more in test group than control group.3.Changes in Improved kupperman score:the total mean score of both two groups after treatment were decreased than before (P<0.05). the mean effective rate of test group was 85.714%,51.852%in control group. Efficacy index difference between the two groups was significant (P<0.05). therpy efficiency had statistical difference between the two groups (P<0.05).4. Changes in singal symptom:the improvement of the symptoms of suspicion, depression, fatigue, musculoskeletal pain, intercourse pain and urinary symptoms in the test group after tretment was not obvious than before (P> 0.05), but tidal fever with sweating, paresthesia, insomnia, irritability, dizziness, headache, palpitation and skin formication improved significantly(P<0.05); the control group also had a significant improvement in tidal fever with sweating, paresthesia, irritability, dizziness and headache (P<0.05), while no significant improvement in other symptoms(P> 0.05). comparison of the two groups after treatment showed that the test group improved in tidal fever with sweating, paresthesia, irritability, fatigue and palpitation (P<0.05), no improvement in other symptoms (P> 0.05).5. Changes in Blood glucose and lipid:Blood glucose in each group after treatment was slightly lower than before, but no statistical sinificance (P> 0.05); blood glucose compared between the two groups after treatment with no statistical significance (P> 0.05). blood Lipid HDL-C in each group after treatment was higher than before (P<0.05), LDL-C in test group after treatment were significantly lower than before (P< 0.05); but changes in TC, TG were not obvious (P> 0.05);comparison between the two groups shows that TC, TG, LDL-C, HDL-C had no statistical significance (P> 0.05).6. Hormone changes in women: compared with before, E2 in test group increased, while serum FSH decreased after treatment (P <0.05); serum LH after treatment had no significant difference than before (P> 0.05). E2, FSH, LH in Control group, had no significant change (P> 0.05). by comparing these two groups, E2 was significantly higher in test group than control group after treatment (P<0.05), while FSH, LH had no statistical significant (P> 0.05.)Conclusion:Claming the liver to suppress Yang and nourishing the liver and kidney decoction combined with western medicine can lower blood pressure better in perimenopausal women with hypertention than western medicine only, the former can significantly improve the clinical symptoms, lower the heart rate, improve blood lipid metabolism and regulate estrogen levels in the body, whose clinical effect is good.
