Objective: To evaluate the therapeutic efficacy of Qu-You-Ding on acne and to probe the underlying mechanism, both clinical observation and anti-inflammatory experiments were performed.Methods: Firstly, in the section of clinical observation, 88 patients with acne vulgaris were randomly divided into Qu-You-Ding group and traditional treatment group. 44 patients in Qu-You-Ding group were topically treated with Qu-You-Ding, while 44 patients in the traditional treatment group were topically treated with Mei-Lu-Xiao-Cuo Ointment. Simultaneously, modified traditional Chinese medicine Pi-Pa-Qing-Fei Decoction and Coptidis Decoction for Detoxification was prescribed for peroral administration in both groups. Acne lesions were observed once a week and the therapeutic efficacy was evaluated 4 weeks later. Secondly, in the section of pharmacological experiment, two kinds of animal models of acute experimental inflammation and one of chronic experimental inflammation were used to examine the anti-inflammatory effects of Qu-You-Ding, including Xylene induced ear edema in mice, egg white induced hind paw edema in rats, and cotton ball induced granuloma hyperplasy in mice.Results: Firstly, the clinical trial showed that 40 patients in Qu-You-Ding group completed the 4-week trial, including 9 cases (22.5%) were cured, 12(30%) remarkably effective, 17(42.5%) effective, 2(5%) ineffective. The total effective rate was 95%. While in the traditional treatment group, 42 patients completed the trial, including 3 cases (7.14%) were cured, 8(19.05%) remarkably effective, 23(54.76%) effective, 8(19.05%) ineffective. The total effective rate was 80.95%. The difference of effective rate between two groups was statistically significant (P<0.05). Secondly, the pharmacological experiments showed that middle dose Qu-You-Ding could evidently reduce the ear edema in mice induced by Xylene as compared with the control group (P<0.01), while high dose Qu-You-Ding had certain anti-inflammatory effects on above three kinds of animal models of experimental inflammation as compared with the control group (P<0.05).Conclusions: Firstly, Qu-You-Ding is more effective than Mei-Lu-Xiao-Cuo Ointment in the treatment of acne vulgaris. Secondly, Qu-You-Ding has certain anti-inflammatory effects on acute and chronic inflammation. Lastly, Qu-You-Ding could treat acne vulgaris by its anti-inflammatory action.
