Objective:General evaluate the clinical curative effect and security by performing the clinical study of Anti-acne cream in treatment ofⅠ~Ⅱgrade acne vulgaris according to Pillsbury standard;To study the mechanism of anti-acne cream on experimental acne model in rabbits in affecting rabbits ear skin,T,E2 level and hemorheology,and on experimental imflammation model in mouse in affecting inflammatory reaction as well.Method:The clinical research 1.Divide the patients who are suffering fromⅠ~Ⅱgrade acne vulgaris according to Pillsbury standard with the ingression standard into two groups randomly.2.The treatment group includes 33 cases smearing anti-acne cream once a night for 42days.The control group includes 31 cases, smearing 0.025%retinoic acid cream.once a night for 42days.3.Obsering indicatrix:to record the number of exanthema papulosum,agria,comedones ect separately evey two week,and take notes of adverse reactions Clinical criteria reference to the efficacy criteria on "efficacy criteria and grading standard of acne vulgaris" instituted by Chinese Medical Association dermatology and cosmetology group Oct.1997.The experiment research 1.Anti-acne cream in affecting rabbits ear skin,T,E2 level and hemorheology on experimental acne model in rabbits.(1)the establish of acne model:The male albino rabbits were used.According to Kligmans method,coal tar was applied daily to the area nearby the opening of albino rabbit ear canal about 2cm×2cm ambit for two weeks.(2)Grouping and administration:according to macroscopic observation score and weight the rabbits were divided into seven groups.Normal group and model group were smeared 0.13ml/kg N.S;Anti-acne cream of high dosage,middle dosage and low dosage group were smared anti-acne cream 0.08g crude drug /kg.d,0.04g crude drug /kg.d,0.02g crude drug /kg.d separately,base material of anti-acne cream group were sweared base material of anti-acne cream 0.13g/kg.d,control group were sweared 0.025%retinoic acid cream 1.4×10-5g/kg.d.Obsering indicatrix:The acne keratinous model were observed by nakedeye and pathological histoligical examination also T,E2 level and hemorheology as well.2.The influence of anti-acne cream on the extent of ear swelling The ear swelling model were caused by the smear of croton oil on ear inner surface in mice. 0.2g crude drug /kg.d,0.1g crude drug /kg.d,0.05g crude drug/kg.d of anti-acne cream were smeared respectively on ear outside surface once for three days.Control group were sweared 2.67g base material/kg.d),complex dexamethasone cream 1.17×10-4g/kg.d,2.67ml/kg N.S once for three days.The extent ofear swelling was measured.3.The influence of anti-acne cream on leukocytoplania model:leukocytoplania model were caused by carboxymethyl cellulose,0.2g crude drug/kg.d,0.1g crude drug /kg.d,0.05g crude drug /kg.d of anti-acne cream were smeared respectively on baldness surface once for three days.Control group were sweared 2.67g base material/kg.d),complex dexamethasone cream 1.17×10-4g/kg.d,2.67ml/kg N.S once for three days.the number of WBC in carboxymethyl cellulose in mice was measured under microscope.Result:The clinical research There is no significant difference between the treatment groupand control group of synthetic curative effect;There is a significant difference between the treatment group after treating 2 weeks and 4weeks,but there is no significant difference between the treatment group after treating 4 weeks and 6weeks;there is no significant difference between the treatment group and control group of total skin rash score and comedones score.Aftering treating 2 weeks,there is a significant difference between two groups in exanthemapapulosum and agria;The safety indicators show the anti-acne cream is safetyThe experiment research 1 Anti-acne cream in affecting rabbits ear skin,T,E2 level and hemorheology on experimental acne model in rabbits:As to macroscopic observation score,there is a significant difference between anti-acne cream of high dosage,middle dosage,low dosage group and 0.025%retinoic acid cream group;low dosage group and high dosage make no significant difference,but there is a significant difference between high dosage,low dosage group and middle dosage group.As to pathological histoligical examination score,there is a significant difference between anti-acne cream of high dosage,middle dosage,low dosage group and 0.025%retinoic acid cream group;there is a significant difference between high dosage group,low dosage group and middle dosage group.There is no significant difference between each group concerning T,E2 level.There is significant difference.Concerning whole blood viscosity,There is a significant difference between anti-acne group(high middle low dosge) and normal group,model group;but there is no significant difference between each group concerning plasma viscosity.2.The influence of anti-acne cream on the extent of ear swelling:Comparing with normal group and base material group,there is a significant difference between anti-acne cream group.And there is also a significant difference between anti-acne cream group and complex dexamethasone cream group.3.The influence of anti-acne cream on leukocytoplania model:Comparing with normal group and base material group,there is a significant difference between anti-acne cream group(except low dosage group).And there is also a significant difference between anti-acne cream group and complex dexamethasone cream group.Conclusion:In treatment ofⅠ~Ⅱgrade acne vulgaris according to Pillsbury standard there is no significant difference between anti-acne cream and 0.025% retinoic acid cream,and after treating 2 weeks,anti-acne cream is better to deal with exanthemapa pulosum and agria;after treating 4 weeks,to increase the time of therapy may have no benefit to the skin rash.;The mechanism of anti-acne cream is related to its anti-cornification and anti-inflammatory effect,and perhaps concerned with decreasing whole blood viscosity.
