"Excessive internal heat" is a common saying when people appears the symptoms like ulcer on the mouth and tongue, swollen and painful gum, constipation and so on. This conception is derived from the body and disease conception in traditional Chinese medicine and therefore doesn't have homologous nomenclature in modern medicine and also doesn't have objective physicochemical indicators.Along with the introduction of the conception of sub-health and the attention of prophylactic medicine, it is significant to have a systemically collating and mining the experience and cognition which belong to the category of sub-health. This research use the method of Epidemiological Investigation to make a preliminary discussion of the symptoms,the tongue demonstration,the body constitution and the Correlation with the physicochemical indicators of the"excessive internal heat"from the angle of"preventing disease before taking bad in TCM". This thesis is composed of the following two parts:Theory research1 To summarise the records about the"excessive internal heat"in the novels and notes in Ming and Qing dynasty and to conduct a literature reviews on the morden literatures about"excessive internal heat".After this, to explore the concept, cause, pathogenesis, symptoms and prevention of"excessive internal heat"2 Having a systematic collation about the theory of"fire"in TCM and discussing the relation between"excessive internal heat"and the"fire"in TCM.Clinical research1 A respective survey was conduct in 758 body-check people. The results showed that there are 91% people have the experience of"excessive internal heat", and the current incidence rate was 4.2%, Female especially the aged were not only more easier to suffer from"excessive internal heat"but also with longer duration than male. There was a significant difference between the gender about some symptoms of the"excessive internal heat". From this investigation,we acquired the initial epidemiologic and symptomatic features of"excessive internal heat".2 Using the case-control method to do a research about the symptoms, tongue demonstration, body constitution and the body-check indicators of"excessive internal heat".2.1 Symptoms2.1.1 The symptom characteristic The results showed that the symptoms significantly associated with"excessive internal heat"including: dry mouth, thirsty with desire to drink, restless emotion, yellow urine, sore throat, sore gums of the teeth, acne, halitosis, bitter taste of mouth, fidget, irritability, constipation, dizziness, yellow phlegm, aphtha, gum in the eyes increase, headache, perleche, red swelling of the eyes and nose bleeds. 2.1.2 The differences of the symptoms between gender The results showed that The Symptoms apt to occur with male are aphtha, halitosis while thirsty with desire to drink, restless emotion, fidget, acne, constipation are apt to occur with female.2.2 Tongue demonstration2.2.1 The tongue characteristic by macroscopic observationThere were no statistic significant of the togue color,thickness of tongue fur,coating color,moistness of tongue fur in different gender and age.The structure proportion suggest that the control group to the maximum extent manefest the nomal togue demonstration like light red tongue, thin white coating, moderate moistness of togue.By contrast to control,the tongue characteristic of"excessive internal heat"by macroscopic observation manifest as red color in the middle and edge of the togue,thick fur in the middle and root of togue, light yellow fur, dry fur and increased prickles on tongue tip(P<0.05).2.2.2 The tongue characteristic of digitization tongue pictureThe tongue characteristic of digitization tongue picture of control group manefest as:the L* value(light) of the tip tongue displayed downward trends in female while the crack index displayed upward trends in male along with the growing of age(P<0.05).the a* value(greenâ†'red) of the tip and edge of tongue was bigger Whereas the L* value and the b* value(blueâ†'yellow) were lower in male than in female(P<0.05).the humidity of tip and root of tongue and the crack index of male were bigger than femal(P<0.05).By controling age and gender,the tongue charactristic of"suffer from internal heat"manefest as :the b* valueon the edge of tongue was bigger while theâ–³a* valuewhich means color difference between tip and edge of tongue was smaller than control in L*a*b* colorspace(P<0.05).the H* value(hue) on the tip of tongue and the C* value(saturation) on the edge of tongue were bigger than control in L*C*H* color space(P<0.05),and the humidity of the tip and root tongue and coating area were smaller than control group(P<0.05).2.3 Body-check indicatorsBy contrast to control,the results showed that body mass index (BMI), uric acid (UA) were higher whereas urine specific gravity (SG) was lower in"excessive internal heat"group(P<0.05). There was statistical significant when compared the UA which are abnormal in each group (P<0.05, OR=4.404). The results of binary logistic regressing showed that genderse,age,UA, low-density lipoprotein(LDL-C)were positively correlated with"excessive internal heat"whereas high-density lipoprotein (HDL), urinary vitamin C,blood urea nitrogen (BUN) were negatively correlated with"excessive internal heat"(P<0.05).Conclusion:1"Excessive internal heat"is one kind of sub-health state which posses of light heat characteristics, correcting the unhealthy lifestyle would assistant to the relief and prevention.2 The symptoms of"excessive internal heat"make differences between genders. Knowing the symptom characteristics and the gender differences will contribute to adjust the"excessive internal heat".3 The tongue characteristic of"excessive internal heat"by macroscopic observation manifest as red color in the middle and edge of the togue,thick fur in the middle and root of the togue, light yellow fur, dry fur and increased prickles on tongue tip.4 The tongue characteristic of digitization tongue picture manifest as incresed H*value(Hue) on the tip and edge of tongue, incresed C*value(saturation) and b*value(blueâ†'yellow) on the edge of tongue, decreasedâ–³a* value which means color difference between tip and edge of tongue (greenâ†'red)ecreased humidity of the tip and root tongue and coating area.5 There are consistency between digitization tongue picture and tongue demonstration by macroscopic observation. Although the digitization tongue picture is more accurate on the objective and quantitative.it still cannot completely replace macroscopic observation with eyes.6"Excessive internal heat"is correlated with body constitution and metabolic disorders.it may be the manefestation of the heavy burden of kidney.
