| ObjectiveTo investigate the clinical results of oral and external Chinese traditional medicine to treat humeral shaft fracture after operation.MethodsFrom June 2002 to March 2004, cases were selected according to the following standards: (1)Older than 18 years; (2)Humeral shaft fracture; (3) Attention to injury of nerves and blood vessels; (4) Inefficacy for conservative treatment, having definite operative indications without no operative contraindications. 160 cases coincided to above standards; they were older than 18 years, including 72 men and 48 women; according to "The standard of Chinese traditional medical diagnosis and curative effect", were divided into up1/3 fracture 46 cases, middle 1/3 fracture 62 cases, down 1/3 fracture 52 cases. According to AO classification, there were 42 cases fracture of type A, 84 cases of type B and 34 cases of type C. They were randomly divided into two groups, and each group has 80 cases treated with two different methods. There was no significant difference of sexuality, age, injury site, fracture classification and internal fixation methods between two groups and has comparability.80 cases of the treatment group were treated with oral and external Chinese traditional medicine after internal fixation combined. (1) Operation was performed under brachial plexus or general anesthesia. Internal fixation methods of interlocking medullary nail (IMN), dynamic compression plate and limited contact-dynamic compression plate (LC-DCP) were selected according to economical condition and fracture type of patients. But fracture type of A or B interlocking medullary nail was not used for open fracture as far as possible to avoid osteomyelitis after operation. (2) oral and external Chinese traditional medicine to treat humeral shaft fracture after operation. During the early period, the main purpose of medicine is activating blood to dissipate blood stasis,alleviating swell and pain, heat clearing and nounshing yin. We prescribe Chinese ready-made drugs made by our own hospital,the oral fulid for calming fracture, 50～100ml per time, three times a day. During the middle period, the main purpose of external drugs is rejointing the bone and tendon, the ointment of rejointing bone, 50～100ml pertime, three times a day. Functional exercise with Chinese traditional medicine. Extension and flexion exercise of fingers of injured site was performed at 3 days after operation. X-ray re-examination was done every one month after operation, and external fixation plaster was removed until continuous bony callus grew through fracture site and the fracture line was blurredAfter the splint being removed away, we used Chinese traditional medicine to fume and wash, to soothe tindon and activate channel. 30～40 minutes per time, two times a day.80 cases of the control group were treated with internal fixation combined. After operation with tablet calcium to treat. X-ray re-examination was done every one month after operation, and external fixation plaster was removed until continuous bony callus grew through fracture site and the fracture line was blurred. Let functional exercise.The above 160 patients were all followed up and the follow-up period was from 6 months to 24 months (12 months averaged). X-ray re-examination was done every one month after operation to observe and compare bony callus growth, bone healing period, recovery of joint function of extremities and the rate of bone late healing and nonunion.ResultsTo evaluated according to standard draft of fracture's recovering effection of the National Traditional Medicine Conservancy＜the diagnosis and effection standard of traditional medicine＞and according to Chinese Hand Society Chinese Medical Accociation (2005-3-25). The total effectual rate of therapeutic team was 97.5%, the total effectual rate of compared team was 82.5%. the fracture healing period was shorter(P＜0.01), the rate of excellent and good cases was higher(P＜0.01), and the rate of bone late healing and nonunion was lower for the treatment group(P＜0.01).ConclusionsWith oral and external Chinese traditional medicine after internal fixation combined treatment humeral shaft fracture has advantages of shorter fracture healing period, higher rate of one-stage healing, good recovery of joint function of extremities and lower rate of bone late healing and nonunion after operation. It has few complications and sequela, relieve economical burden of patients and society and is easy to be accepted by patients. So this method deserves to be spread by clinicists. |
