Humeral shaft fracture was a common fracture,accounting for about 1-5% body fractures[1,2].With the modern,mechanized process,humeral shaft fractures,especially in high-energy fractures caused by,became upward trend evidently.Most of the humeral shaft fracture did't required surgery[3]. They can be cured by conservative treatment.However,as a result of work pressure increasing,the pace of life speeding up in recent years,the demands for the fracture recovery increased than before.Coupled with income in- creasing,the insurance system improving, the improvement of internal fixa- tion and transplant,the surgery treatment was gaining popularity.The surgical indications were wider than before[4,5].The surgical treatment be- came the main methods in humeral shaft fracture,even than the conservated treatment,such as manipulative reduction and plaster,bandages, splints and external brace,had some advantages.Recently,the main surgical treatment was the intramedullary nail and the plate,although a variety of external fixator was applied.Either intramedullary nail or plate had positive effects.Objective:The aim of this study was to compare and investigate the clinical effects of the two newly devices,which was unreamed humeral nail (UHN) and locked compression plate (LCP),for the management of diaphyseal fractures of the humerus.This study could made the surgical treatment of humeral shaft fracture better.Methods:79 humeral shaft fracture patients,who were adult fresh closed fractures,admitted from January 2003 to December 2008 in our hospital, were reviewed retrospectively.48 males and 31 females were included.The average age was 48.48(19-74) years old.They did the opera- tion in 2 weeks after injured.According to different materials,they were divided into UHN (26 cases) and LCP (53 cases) group.Group UHN were treated by way of unreamed and antegrade.Group LCP patients were mostly operated through lateral,anterolateral and posterior approach,according to their fracture location.Postoperative follow-up with the strict conditions,the positive function of active and passive shoulder and elbow rehabilitation exercises were given.Operation time,blood loss,intraoperative com- plica- tions, fracture healing and the postoperative recovery of shoulder and elbow function was compared.79 cases of all patients were followed by 3-45 months, the average 11.5 months,Group UHN 10.8 months,Group LCP 11.9 months.The statistical analysis software SPSS13.0,One-way ANOVA and Fisher's Exact Test was used for statistical analyse.P<0.05(bilateral)was considered significantly.Results:All cases were followed-up for 3~45 months,the average 11.5 months.No infection and fixed failure occurred.The intraoperative comp- lications,fracture healing,functional recovery of shoulder and elbow of the two groups were almost the same.The operation time,blood loss of group UHN was better than the group LCP(P<0.05).Conclusion:UHN and LCP had the same therapeutic effect at last.They were the ideal methods for internal fixation for humeral shaft fracture.UHN can be used as first choice in the patients with poor tolerance for operation, because of it less operation time and less blood loss.
