Flavored Dachaihutang The Treatment Of Patients With Early Severe Acute Pancreatitis

Objective:Through clinical study on the treatment of early Severe Acute Pancreatitis (SAP)with Jia-Wei-Da-Chai-Hu soup, analyse it' s effection to the level of endotoxin. TNF-a Il-6 and SOD in the patients' blood. To explore the function machanism of the Jia-Wei-Da-Chai-Hu soup to severe Acute Pancreatitis. MethodThe 50 early SAP case who have been made a definite diagnosis were divided into two groups at random. Integrated traditional Chinese herbal soup and western medicine group 9treatment group), western medicine group (contrast group). Western medicine therapy include heavy sickness guardianship, giving oxygen, fasts, stomach and intestines decompression, supplement of blood capacity, maintenance of acid-alkali balance and water-electrolyte balance ,using restraining enzyme medicine and antibiotic, giving nutrition support, spasmolysis and stopping pain and so on;Case with multi-organs funtion failure giving organ function support. On the basis of the above , the treatment group add Chinese herbal soup pouring from stomach tube and rectum. In the day before treatment and the first day, the third day and seventh day next, each case is observed general clinical symptom and abdomen symptoms alleviating time, intestinal tract function restoring time, and measured blood amylase,plasma endotxion, TNF-a, IL-6 level, the serum S0Dlevel;and observed APACH-2 marks, Balthazar CT grades and pancreatic infection incidence in the day before treatment and the seventh day next.the make evaluation of curative effect. Result(1)Compare with contrast group , it can improve the generalclinical symptom faster using integrated the Chinese herbal soup and western medicine, decreasing the time of intestinal tract function restoring time and increasing the times of defecate, with having remarkable difference (p<0.05);(2)Jia-Wei-Da-Chai-Hu soup can obviously reduce the plasma endotoxin level, reduce TNF-a, IL-6 level, remove the free oxygen factor.(3)The blood amylase level reduced obviously in two groups .It dropped faster in the treatment group . (p<0.05)(4)Both of two groups can obviously reduce APACHE-2 marks and Balthazar CT grades (p<0. 01), the treatment group reducing more obviously(p<0. 05)(5)The pancreatic infection incidence of the treatment group is lower compared to that of the contrast group(p<0. 01) ConclusionJia-Wei-Da-Chai-Hu soup can alleviate notably endotoxim blood disease and the cell factor releasing caused by it .relieve the abdomen . symptoms of SAP patients rapidly, improve gastrointestinal tract function, prevent and treat pancreatic infection and necrosis. As it show, integrated Traditional Chinese and Western Medicine is better than the simple comprehensive treatment of Western Medicine. The Jia-Wei-Da-Chai-Hu soup has played role of overall adjusting control and multiple treatment .
