First Part Effect of Chai Huang Qing Yi Huo Xue Granule on Serum Related Inflammatory Factors in Patients with Severe Acute Pancreatitis Objective:To observe the effect of Chinese medicine Chai Huang Qing Yi Huo Xue Granule on inflammatory factors in patients with severe acute pancreatitis,and to provide evidence for clinical application and further study of this drug.Methods:From the inpatients of the Hospital of Gastroenterology from January 2017 to April 2018,30 patients with severe acute pancreatitis who met the inclusion and exclusion criteria were randomly divided into two groups,the experimental group and the control group.15 patients in each group,the control group was given basic treatment,and the experimental group was fed with Chai Huang Qing Yi Huo Xue Granule tube(or oral)and enema.Blood samples were collected from patients at admission(0 days),3 days,and 7 days.Serum amylase was detected by automatic biochemical analyzer.The levels of tumor necrosis factor-?,interleukin-8 and nuclear factor-?B were detected by double-antibody sandwich enzyme-linked immunosorbent assay.Results:(1)In the control group and the experimental group,serum AMY,TNF-?,IL-8 and NF-?B were increased at admission(day 0),and there was no significant difference between the two groups(P>0.05).Serum inflammatory factor levels were comparable between the two groups.(2)AMY,TNF-?,IL-8 and NF-?B were significantly decreased on the 3rd and 7th day of the two groups compared with the admission(0 days),the difference was statistically significant(P<0.05)..On the 3rd and 7th day of the experimental group,AMY,TNF-?,IL-8 and NF-?B were lower than the control group,and the difference between the groups was statistically significant(P<0.05).Conclusion:(1)In the basic treatment,the addition of Chai Huang Qing Yi Huo Xue Granule can significantly reduce serum amylase in patients with severe acute pancreatitis.Combined with the results of previous clinical trials,Chai Huang Qing Yi Huo Xue Granule can improve the curative effect.(2)In the basic treatment,the addition of Chai Huang Qing Yi Huo Xue Granule can significantly reduce the serum levels of NF-?B,TNF-? and IL-8 in patients with severe acute pancreatitis,suggesting that Chai Huang Qing Yi Huo Xue Granule can reduce the inflammatory factors associated with severe acute pancreatitis.Levels,thereby reducing the inflammatory response,further control the development of the disease,and its anti-inflammatory mechanism may be related to the NF-?B signaling pathway.The Second Part TW0 Effect of Chai Huang Qing Yi Huo Xue Granule on IKK/I?B/NF-?B Signaling Pathway in Rat Model of Severe Acute PancreatitisObjective: To observe the effect of Chai Huang Qing Yi Huo Xue Granule on the expression of I?B kinase,NF-?B inhibitory protein ?,phosphorylated NF-?B inhibitory protein ? and nuclear factor ?B in pancreatic tissue of SAP model rats,in order to To explore the possible mechanism of action of Chai Huang Qing Yi Huo Xue Granule in IKK/I?B/NF-?B signaling pathway.Methods:(1)Grouping: 48 rats were randomly divided into 3 groups,namely SAP model group,sham operation group and experimental group.(2)Modeling: Rat model of SAP was prepared by retrograde injection of pancreaticobiliary tube with 3.5% sodium taurocholate at a dose of 1ml/kg.The rats in the sham operation group were opened,and the abdominal organs were turned over.belly.(3)Dispensing: The Chai Huang Qing Yi Huo Xue was formulated into a drug solution having a concentration of 0.16g/ml.(4)Administration: The experimental group was intragastrically administered with a solution of 10 ml/kg/time,once every 4 hours(not administered by 0-6);the sham operation group and the SAP model group were orally administered with the same dose of normal saline.(5)Materials: 8 rats were sacrificed at 12 h and 24 h after operation,and 8 rats were sacrificed.Gross anatomy was observed,blood was taken from the celiac artery,and pancreatic tissue was completely removed.(6)Detection: serum amylase and lipase were detected by automatic biochemical analyzer;the expression of IKK? and I?B? m RNA in pancreatic tissue was detected by real-time quantitative PCR;the expression of IKK?,I?B? and p I?B? protein in pancreatic tissue was detected by Western blotting.The expression of NF-?Bp65 in pancreatic tissue was detected by immunohistochemistry.Results:(1)Compared with the sham operation group,the serum AMY and LIP activities of the SAP model group were increased,the difference was significant(p<0.05);the serum AMY and LIP activities of the experimental group were decreased compared with the model group for 12 h,24h.The activity was lower and the difference was statistically significant(p<0.05).(2)The expression of IKK? m RNA in pancreatic tissue of SAP model group was higher than that in sham operation group(p<0.05).Compared with SAP model group,the expression of IKK? m RNA was decreased at various time points in the experimental group.The significance of the study(p<0.05);compared with the SAP model group,the expression of I?B? m RNA in the pancreatic tissue of the experimental group increased at each time point,the difference was statistically significant(p<0.05).(3)The expression levels of IKK? and p I?B? in the SAP model group were significantly higher than those in the sham-operated group(p<0.05).Compared with the SAP model group,the expression levels of IKK? and p I?B? in the pancreatic tissue of the experimental group decreased.There was significantness(p<0.05).Compared with the sham operation group,the expression of I?B? protein in pancreatic tissue of SAP model group was lower,the difference was statistically significant(p<0.05).Compared with SAP model group,the pancreatic tissue of experimental group was used at different time.The expression of I?B? protein was increased at the point,and the difference was statistically significant(p<0.05).(4)The positive expression and score of NF-?Bp65 in pancreatic tissue of SAP model group and experimental group were significantly higher than those in sham operation group(p<0.05).Compared with SAP model group,the experimental group was 12 h,24h pancreas.The positive expression and score of NF-?Bp65 were decreased,and the expression was lower at 24 h than at 12 h,the difference was statistically significant(p<0.05).Conclusion:(1)The serum amylase and lipase of the SAP model group were elevated,and the results of the previous study combined showed that the model was successful.(2)The expression of IKK?,p I?B? and NF-?Bp65 in pancreatic tissue of rats in SAP model group was increased,and the expression of I?B? was decreased,suggesting that IKK/I?B/NF-?B signaling pathway may be activated in severe acute pancreatitis,activation and activation of IKK?.There is a relationship between phosphorylation of I?B?,which may cause the release of inflammatory mediators and aggravate the inflammatory response.(3)Chai Huang Qing Yi Huo Xue Granule has up-regulated the expression of I?B?m RNA and I?B? protein in pancreatic tissue,and down-regulated the expression of IKK?,p I?B? and NF-?Bp65,suggesting that Chai Huang Qing Yi Huo Xue Granule can inhibit IKK/I?B/NF-?B signaling pathway in severe acute pancreatitis.Reducing the release of inflammatory mediators may be one of the mechanisms for its treatment of SAP.
