Objective To study the clinical effect of application with ChaiqinChengqi Decoction in the early phase of severe acute pancreatitis(SAP). Method 120 patients with SAP were allocated at random into the treatment group and the control group with the ratio of 2:1 in their early stage (72 hours) . The therapy of ChaiqinChengqi Decoction with integrated traditional Chinese medicine and western medicine were used in the treatment group at admission. While ChaiqinChengqi Decoction was applicated 3 days after admission in the control group.Laboratory parameters such asWBC,ALT,AST,,Urea,CREA, AMY, LIP and urine AMY/Cr, were examined on days 1, 3 and 7 ; scores of Acute Physiology and Chronic Health Evaluation score II (APACHE II )are also compared at the same time. Infection ,local complications and organ/systemic complications, the rate of ICU-treated patients,hospital stay, surgical operation rate and mortality rate were compared between the two groups. Data on the 120 patients subjected to further logistic regression analysis consisted of age, gender, admission to ICU , infection, need for mechanical ventilation, scores of APACHE II at admission , intestinal paralysis relief time , organ failures and so on . Results There were significant differences in the rate of infection, gastrointestinal and respiratory failure lasting time between the treatment group and the control group.But no significant differences were noted whencomparing the early mortality and the overall hospital mortality rate during the study. By multivariate logistic regression analysis, the significant risk factors for developing infection were intestinal paralysis relief time, treatment in ICU,and the course of treatment with mechanical ventilation.Conclusions Application with ChaiqinChengqi Decoction in the early phase of patients with SAP was an effective therapy to lower the rate of infection, intestinal and respiratory failure lasting time. But it did not offers a survival benefit to patients with SAP compared with those in control group .
