Osteoporosis (OP) is a kind of disease characterized by low bone mineral density(BMD) and microarchitectural deterioration of bone tissue, which leads to enhanced bone-fragility and a consequent increase in the risk of fracture. As one of the commonest metabolic diseases,Osteoporosis has been considered a disease whose incidence of disease, deformity and death rate is high. The cause and pathogenesis of OP are complicated; there are no satisfying drugs in modern medicine for it. However, people are increasingly pay attention to traditional Chinese medicine, for the concept regarding the human body as a whole and its tiny side effect. gu-song-an capsule is a pure preparation of TCM, which has been used for a long time in the department of orthopaedics of GuangDong provincial hospital of TCM. Its effect has been proved to be satisfying by many years?clinical applications, and its side effect is tiny. Based on a survey of the studies about OP by TCM and western medicine, this thesis explores the pathogenesis of OP and the effect of gu-song-an capsule, and carries out this clinical study-in order to know the curative effect of gu-song-an capsule to OP.The first part is studies of correlative literature. In this part we summarize the domestic and foreign studies during the latest several years. The defining and meaning of OP is explained in detail. As is adopted by all, OP can be categorized into primary, secondary and idiopathic these three kinds. Primary osteoporosis(POP) is classified into two types:postmenopausal osteoporosis (PMOP) and senile osteoporosis (SOP). The pathogenesis of it is closely related to body hypofunction caused by senilityand shortage of the secretion of estrogen. In addition, lack of vitamin D receptor(VDR) gene, lack of parathyroid hormone(PTH),decrease of the synthesis of active vitamin D, reduction of the secretion of calcitonin, dysfunction of some factors such as male hormone, growth hormone and some cell factors such as: insulin條ike growth factor(IGF), interleukin(IL) etc, which have relations to the happening of OP too. There is still no universally acknowledged standard for OP diagnosis . BNfD is only a main reference index, and biochemistry indices, such as BGP, PYD, HOP etc., are key reference factors when OP need to be judged. As to the western drugs, there are bone forming accelerant, bone梐bsorbing inhibitor, bone nutriment these three kind of drugs. There is no term for 揙P?in TCM. But the reasons and symptoms of 揋U WEI?GU means bone and WEI means fading) looks like those of OP. According to the theories of 搆idney masters bone?in TCM, OP belongs to primary asthenia of kidney. So invigorating kidney is the therapeutic principles for OP. Besides, acupuncture~ moxibustion and physiotherapy can obtain good effects too. The second part is clinical study. We divide the selected patients randomly into two groups: the treating group and the control group, record their score of symptoms and signs of TCM, blood AKP, urine PYD and BMD before they are treated. And after the treatment we record them again. The result show that before the treatment there are no significant differences between these two groups in score and blood AKP, urine PYD and BMD statistically, which indicates the two groups have comparability. After treatment with gu梥ong--an capsule and Rocaltrol tablet separately, their score and blood AKP, urine PYD and BMD are recorded again, Compared with before all indices have significant differences except BMD index, which6indicates that the two drugs have good effect both. The score index of the treating group is compared with that of the control group, there is also significant difference. And side effect of gu梥ong梐n capsule is much less than Rocaltrol tablets? with similar non梤echecking rate. The detailed clinical data show that all non梤echecking patients have serious OP. So the two drugs should be applied to slight or moderate OP ,not to serious OP. For the patients that has serious OP, we should associate with other treating measures.
