The information age comes with the rapid development and popularity of internet. For the information sources are the most precious fortune in this age, it is quite important for the enterprises to know the latest and useful information in time. As a result, the information constructions are increasingly deepened in plenty of industries.Nowadays, it is quite difficult to make effective data report for different kinds of industries, especially for the enterprise information systems. Thanks to the uncertainty of the needs from the customers and the running environment, dynamic and changeable form is required to be provided by the system to meet the customers'needs while making data reports. However, the existed solutions for dynamic form often can not achieve this because they're usually designed in the implementation layer according to some development technology or system structure. So it becomes so important to make research on the data model of dynamic form in database layer for Management Information Systems. It can not only meet the commands of customers, but also can result in flexibility independent of the development technologies.Firstly, this paper makes inquisition on the demands of dynamic data report under the background of the development project of E-government system for Chongqing Commerce Commission (case E-government system for short). Then the researches of dynamic form technologies home and abroad are made in detail. On the basis of analyzing the shortage of realizing dynamic form on business logical tier,the method of designing dynamic form data model based on virtual table on database tier is presented. This model make storage of the definition and structure description of form in the way of rows in database, which make sure that the structure modification of forms, can be performed in a flexible way. Meanwhile, while designing the storage method of report data, the estimate increment of every data storage table is fully considered. Through calculating and minimizing of estimate variance, the volume of data in every table is kept in average and the efficiency of storage space can be kept in a maximum level. Then, realize the virtual table based dynamic form model in case E-government system successfully, at last, approve the validity and feasibility of this model by the analysis of testing result. Testing result indicate that the dynamic form design and realize project referred by this paper can provide flexible and reliable dynamic data report function for case E-government. This design and realize method has stated reference value to the design of dynamic data report for E-government system and other information management system, especially for the mass demand of report form.
