Baibi, modern clinical medicine known as psoriasis, which is clinically more common chronic inflammatory proliferative skin disease, intractable and easily recurrence. Global prevalence rate in adults was 0.91%-8.5%, in recent years our country Baibi prevalence rate showed a rising trend, the recent epidemic disease investigation showed that the prevalence rate was 0.47%, higher than in previous years, the survey results, and male and female incidence rate of no significant difference in statistics. The etiology and pathogenesis of Baibi is not clear, the literature reports related Baibi pathogenesis to a variety of cytokines, inflammatory cytokines and signaling pathway[Object]To investigate the TOPK, COX2, P38 and JNK of MAPK signal pathway related kinase in Baibi of blood heat syndrome in patients with lesions of the expression to observe the differences in their levels of expression in the skin lesions and normal skin tissue, and to explore the relationship between them, further explore their role in Baibi pathogenesis, to provide new ideas and theoretical basis for the treatment of Baibi.[Methods]Using the SP method of immunohistochemistry were used to detect the expression levels of TOPK,COX2, P38 and JNK in 22 patients with psoriasis of blood heat syndrome in patients with lesions and 10 cases of normal skin tissue. Using image analysis system software for image acquisition, with average density value evaluation of the positive results, and the protein kinase in expression between blood heat syndrome of Baibi and normal skin tissues. All datas were analyzed by SPSS 17.0 software, the positive rate between the two groups was tested by Fisher’s exact probability; the significant comparison between the two groups with normal distribution of the data used t-test, does not meet the normal distribution of the data using the Wilcoxon rank sum test,and correlation analysis used Spearman correlation analysis, test were performed using a two-sided test, test level a=0.05.[The result]1. TOPK, COX2 and P38 protein expression positive rate in the skin lesions of patients with blood heat syndrome of Baibi was significantly higher than that in normal skin tissues (P <0.05); and JNK no significant difference (P>0.05).2. TOPK is moderate to strong positive expression in Baibi of blood heat syndrome in patients with lesions, no expression in normal skin tissues. Expression levels in the skin lesions of patients with blood heat syndrome was 0.30±0.13 and in normal skin tissues the expression level was 0.09±0.02, there was a significant difference between the two (P< 0.05).3. COX2 is moderate to strong positive expression in Baibi of blood heat syndrome in patients with lesions, no expression in normal skin tissues. Expression levels in the skin lesions of patients with blood heat syndrome was 0.26±0.11 and in normal skin tissues the expression level was 0.09±0.02, there was a significant difference between the two (P< 0.05).4. P38 in Baibi of blood heat syndrome in patients with lesions showed weak to strong positive expression and in normal skin tissues were weakly positive expression or no expression. In the skin lesions of patients with syndrome of blood heat expression level was 0.27±0.07, in normal skin tissues the expression level was 0.16±0.04, there was a significant difference between the two (P<0.05).5. The expression of JNK was weakly positive in Baibi of blood heat syndrome of patients with lesions, showed very weak expression or no expression in normal skin tissues in the cytoplasm. In the skin lesions of patients with syndrome of blood heat expression level was 0.23±0.09, and in normal skin tissues the expression level was 0.10±0.02, there was a difference between the two (P=0.05).6. In Baibi of blood heat syndrome in lesions of patients, there was positive correlation between the expression of TOPK and COX2; the expression of COX2 was positively correlated with JNK.[Conclusion]1. TOPK in Baibi of blood heat syndrome in lesional epidermis cells in all layers of the cytoplasm and the nucleus showed moderate to strong positive expression, in the upper spinous layer and granular layer cells mainly expressed in the nucleus, in spinous layer and the basal layer cells was mainly expressed in the cytoplasm, no expression in normal skin tissues. There was a significant difference between the two, suggesting that TOPK involved in the inflammatory process in psoriasis.2. COX2 in Baibi of blood heat syndrome in patients with lesions of the epidermal cells in all layers of cytoplasm and nucleus showed moderate to strong positive expression, in the spinous layer and base layer mainly expressed in the cytoplasm, no expression in normal skin tissues, There was a significant difference between the two, which suggests that COX2 involved in the inflammatory process in psoriasis.3. P38 in Baibi of blood heat syndrome in patients with lesions of the epidermal cells in all layers of cytoplasm and a small amount of nucleus showed a weak to strong positive expression, the upper epidermis cells visible in the nucleus is expressed, in the cytoplasm of normal skin tissue showed weak positive or no expression, There was a significant difference between the two. The results suggested that p38 involved in the inflammatory process in psoriasis.4. JNK in Baibi blood heat syndrome in lesional epidermis of all layers of the cytoplasm and nucleus were weakly positive expression and in the cytoplasm of normal skin tissue were very weak expression or no expression, There was a significant difference between the two, suggesting that JNK through transcription factor to promote the mechanism of gene expression and protein synthesis involved in psoriasis pathogenesis.5. In the skin lesions from patients with Baibi of blood heat syndrome TOPK and COX2 expression was positively correlated, COX2 and JNK expression was positively correlated, indicating that may exist between the direct or indirect regulatory relationships.6. Detoxify cooling medicine may exert its therapeutic effect through inhibition of MAPK signal transduction.
