To Observe The Changes Of Blood Vessels In The Treatment Of Psoriasis Vulgaris(Blood Heat Syndrome) Under Dermoscopy And The Correlation With TCM Syndrome Score

Objective:To evaluate the effecacy of Liang xue Xiao feng San on improving the microvessel of psoriasis by observing the diameter of blood vessels and cross-sectional area under dermoscopy before and after Liang Xue Xiao Feng San treatment;and to explore the correlation between psoriasis blood-heat syndrome and vascular changes,to provide evidence for psoriasis blood-heat syndrome.Methods:From March 2019 to December 2019,the patients with Psoriasis vulgaris blood-heat syndrome who participated in the research on inheritance and innovation of the experience of famous and old Chinese medicine were selected as the research subjects,and Liang xue Xiao feng San was administered orally for 4 weeks,and to record the TCM syndrome scores before treatment,1,2,3,and 4 weeks after treatment,and dynamically observed the vascular diameter and cross-sectional area of skin lesions using dermoscopy;to evaluate The changes of TCM syndrome score and vascular index before and after treatment,analyzed and the correlation between TCM syndromes of blood heat syndrome and vascular changes at the skin lesions.Results:A total of 32 patients with blood-heat syndrome psoriasis were recorded.During the course of treatment,the diameter and cross-sectional area of blood vessels at the skin lesions gradually decreased,and there was a trend of returning to normal.The difference of blood vessel diameter and relative index of vascular sphere area before and after treatment were statistically significant,p<0.05.During the course of treatment,the TCM syndromes of patients gradually improved,and the relative index difference of TCM syndromes score before and after examination and treatment was statistically significant,p<0.05.Through typical correlation analysis,TCM syndrome score was positively correlated with vascular relative index(diameter and cross-sectional area),R=0.632,p<0.05,which was statistically significant.Conclusion:(1)Liang xue Xiao feng San has remarkable effect in improving the symptoms of patients with blood-heat syndrome and in the microvessels of psoriasis.(2)There is a certain correlation between the relative index of TCM syndrome score and blood vessels,indicating that while the TCM syndromes have improved,thediameter of blood vessels in the skin lesions becomes smaller,the cross-sectional area of blood vessels becomes smaller,and gradually returning to normal.And with the change of time,the improvement of blood-heat syndrome symptoms in patients is consistent with the trend of returning blood vessels to normal,But the trend is not exactly the same,and the relative index curve of TCM syndromes is always higher than that of blood vessels in the same period,suggesting that the improvement speed of TCM syndromes is better than that of vascular recovery.Because the correlation coefficient between the two is not high,the vascular index can reflect the degree of "blood-heat" in patients to a certain extent,and the change of blood vessels can only be used as an auxiliary index of blood heat syndrome psoriasis.(3)Dermoscopy,as a commonly used examination equipment in dermatology,has great application prospect in disease diagnosis,prognosis evaluation and observation of curative effect.Combined with this study,it is considered that it has certain clinical significance in the evaluation of the efficacy of psoriasis with blood-heat syndrome.
