Objective:To exploring the correlation between Blood Stasis of knee osteoarthritis(KOA) and cytokines (interleukin-1β, transforming growth factor-β1) and the pathology changes under arthroscopy (synovial hyperplasia,cartilage degeneration,the formation of loose bodies,osteophyte formation),to provide objective and quantitative indicators for the Chinese medicine diagnosis and treatment.Method:Collected 40 cases of Blood Stasis of KOA patients for a prospective study,who were hospitalized department of orthopedics in The People's Hospital of Fujian Province and Shenzhen Pingle orthopedic hospital after Syndrome Differentiation. Collected synovial fluid before surgery, detected cytokines interleukin-1β(IL-1β), transforming growth factor-β1 (TGF-β1) levels with ELASA, while row performed arthroscopic debridement, observed the pathological changes under arthroscopy, include:synovial membrane,articular cartilage, loose bodies, osteophyte. Based on the above datum analysis the correlation between TCM Syndrome-type and compare different degree of pathological changes under arthroscopy and cytokine levels.Results:1. Blood Stasis type osteoarthritis of the knee synovial fluid in patients with IL-1β, TGF-β1 levels were:29.81±4.93 pg/ml,57.79±6.96 pg/ml, and IL-1βand TGF-β1 levels were negatively correlated;2. Blood Stasis type of knee arthroscopy patients were seen in articular cartilage degeneration, synovial hyperplasia, most cases could see the formation of loose bodies, osteophyte formation less likely to see. The articular cartilage degeneration in most of typeⅣ, synovial hyperplasia withⅠ-based;3. The correlation between cartilage degeneration and cytokines:the level of IL-1βofⅣdegree of cartilage degeneration was significantly higher thanⅠ-Ⅱdegree of cartilage degeneration group (P<0.05), the level of TGF-β1 ofⅠ-Ⅱdegree of cartilage degeneration was significantly higher thanⅣdegree of cartilage degeneration group (P<0.05), IL-1βand cartilage degeneration was positively correlated, the correlation coefficient was 0.619 (P <0.05); 4. The correlation between synovial hyperplasia and cytokines:the level of IL-1βofⅡdegree of synovial hyperplasia was significantly higher than the degree of synovial hyperplasia group (P<0.05), IL-1βwas positively correlated with synovial hyperplasia, the correlation coefficient was 0.826 (P<0.05);5. The correlation between osteophyte formation and cytokines:the levels of IL-1βand TGF-β1 of I level osteophyte formation were significantly higher than 0 level osteophyte formation group (P<0.05), IL-1βwas positively correlated with osteophyte formation,the correlation coefficient was 0.619 (P<0.05), TGF-β1 was positively correlated with osteophyte formation, the correlation coefficient was 0.4 (P<0.05);6. The correlation between the formation of loose bodies and cytokines:the level of IL-1βof I level of the formation of loose bodies was significantly higher than the 0 level of the formation of loose bodies group (P<0.05), IL-1βwas positively correlated with the formation of loose bodies, the correlation coefficient was 0.318 (P<0.05).Conclusion:1. The articular cartilage degeneration, synovial hyperplasia take place to all Blood Stasis of knee osteoarthritis patients, vast majority of cases seeⅣdegree of articular cartilage degeneration, I degree of synovial proliferation and loose body formation;2. The levels of IL-1βand TGF-β1 are helpful for Blood Stasis syndrome of knee osteoarthritis and providing the objective and quantitative indicators.
