Abstract:Objective:To investigate the mechanism of the expelling wind and stopping itching in patients with eczema by observing clinical effect and the serum level changes of leukotrieneB4(LTB4),interleuken-4(IL-4),interferon-γ(IFN-γ) of patients before and after treatment. Methods:①78 confirmed eczema patients from inpatient department of dermatology were randomly divided into treatment group (40 patients)and control group(38 patients), and 20 healthy cases were treated as normal group.②40 patients in treatment group were administrated the oral liquid expelling wind and stopping itching at an dosage of 10ml each time and 3 times each day. A period of treatment lasted 14 days. At the same time,10% Calcium Gluconate 20ml and Vitamin C 3g with 0.9%NS 250ml was infused by intravenous 1 time each day. Cetirizine was administrated 10mg orally 1 time each day. In the control group the 38 patients were given the same treatment as the treatment group but the oral liquid expelling wind and stopping itching.③The clinical effect of treatment group was compared with control group, and the effective rate was computed by healing rate and marked effect rate.④The serum levels of LTB4, IL-4, IFN-γof 78 cases in patients with eczema before and after treatment and 20 healthy cases were defected by ELISA. Before treatment the serum levels of LTB4, IL-4, IFN-γof all patients were compared with normal group, and these serum level changes of two groups before and after treatment were also compared.⑤The data was analyzed by SPSS 14.0 software. Results:①The patients' condition score of the treatment group was (12.37±1.46),and that in the control group was (12.37±1.46), the difference between them was not obvious. After therapy the patients' condition score of the treatment group was (2.67±2.35),that in the control group was (4.23±2.94),P<0.01,the difference was obvious. After therapy the effective rate in treatment group was 90.00%, and that in the control group was 60.53%,χ2=5.93, P<0.05.The difference between them was obvious.②Before therapy the serum levels of LTB4(60.92±9.17),IL-4 (85.63±9.62) in all eczema patients were higher than that of normal group, the serum level of IFN-γ(15.17±7.70)was lower than that of normal group, P<0.01.③After therapy the serum levels of LTB4 (34.48±14.20),IL-4 (39.37±11.60) in treatment group significantly dropped, the serum level of IFN-y(27.04±10.83) significantly rose, P<0.01,the difference was obvious, but the serum levels of LTB4(62.88±14.99),IL-4(75.74±11.17),IFN-y(17.51±7.28) in control group had no noticeable change, P>0.05. Conclusion:①The clinical effect of treatment group was better than that of control group. It shows that the oral liquid expelling wind and stopping itching can significantly improve swelling and itching and has a clear effect on eczema.②The serum levels of LTB4,IL-4 in patients were higher than that of normal group, and the serum level of IFN-γwas lower. This shows that the three have been involved in the pathogenesis of eczema.③The oral liquid expelling wind and stopping itching contains a variety of effective anti-inflammatory ingredients, these ingredients can reduce the production of LTB4,thus inhibit chemotaxis of neutrophil and a variety of white blood cells and reduce inflammation. At the same time the oral liquid expelling wind and stopping itching contains a variety of substances of modulating immune activity, these substances can regulate the Th1/Th2 cytokine balance and make imbalance of Th1/Th2 network back to normal, thus participates the anti-inflammatory process.
