BackgroundCommunity health resources are the basis of community health service (CHS); Human resource plays the key role in community healthcare. The rational distribution of community human resource will determine the function, quality and sustainable development of community healthcare services. Therefore, it is very important to understand the current situation and the developing process of community human resource in order to improve community health service.ObjectiveTo understand the current situation of human resource in CHS in Zhabei district; to summarize the developing process of human resource in CHS and analyze the existing problems; to provide recommendations to optimize the health workforce of CHS and improve the distribution and training of health workforce in CHS.Method1. Literature and policy review:Relevant literatures and policies were collected, reviewed and summarized.2. Quantitative study:Understand the amount, distribution, structure and quality of human resources by questionnaire.3. Qualitative study:Understand the experience, problem and bottleneck of human resources training and management by in-depth interviews. And the method of framework analysis was used.Findings1.73%-75%of health technicians in CHS centers in Zhabei district were female.2. Most health technicians in CHS centre in Zhabei district graduated from junior college or high school.3. More than 90% of general practitioners are western-based physicians. The amount of general practitioners can not meet the need according to disposition standard in Shanghai.4. Population based gini-coefficient of community human resource was between 0.2 and 0.3. And it was getting better in past three years. Geographic based gini-coefficient of community human resource was between 0.45 and 0.5, which is in warning condition. And the balance of human recourse in health care area has to be improved.5. Zhabei district use "533" training mode to improve the quality of human recourse in CHS centers. It tried to improve the quality of community human resource by following measures:Build up training platform for general practitioners; Put the emphasis on training five-star general practitioner; Enhance personal selection system to; Explore the flowing system of talent.6. Zhabei district paid attention to the management of community human resource of CHS to improve satisfaction of medical staff and CHS talent training system.ConclusionsThe amount of health workforce in CHS in Zhabei district is becoming larger, but still can not meet the demand of aging of population. The situation of human resource of CHS has been improved, but there is still a gap between the current situation and the standard. The quality of health workforce in community health care area is becoming better, but the structure still needs to be improved.RecommendationsPromoting favorable recruit and retain policies. Improving the quality of health workforce in CHS in Zhabei district. Enhancing personnel system reform and increasing the number of senior professionals. Perfecting scientific training system and encourage policy. |
