Objective: Psoriasis is a disease of chronic, recurrent and inflammatory skin disease characterized by epithelial hyperplasia, differentiation out of control and the infiltration of lots of T cells. It is not chear about the pathogenesis of psoriasis; but it is influenced by heredity, metabolic disorders, neuropsychology and so on. It is considered that the disorder of immune plays an important role in the pathogenesis of psoriasis. Studies find that T cell subsets and cytokines are very important in the pathogenesis and treatment in the autoimmune diseases. Interleukin (IL) is a whole class cytokines with a variety of biological effects produced by activated lymphocytes and dendritic cells. Lots of research shows that there is a network of mutual coordination and antagonism between IL-12 family members and other cytokines. It plays an important role n in the cell infection and inflammation and is closely related with the attack of many diseases, such as psoriasis, multiple Sclerosis, Crohn's disease.So far as we know , IL-12 family has four members: IL-12, IL-23, IL-27 and IL-35. IL-12, IL-23 and IL-27 are mainly produced by dendritic cells, macrophages and B lymphoblastoid cells, and IL-35 mainly by the Treg, which is also generated by B cells transformed by EBV. IL-23 is comprised by two subunits of p19 and p40 of IL-12, which are linked by disulfide bond. IL-23 is produced mainly by activated monocytes and macrophages (Mφ) and dendritic cells (DC). In the past few years, there are many studies on the relationship of IL-23 and lesions of psoriasis, and the major research direction is the correlation studies between the level of IL-23 in the lesions of psoriasis and the severity of psoriasis and the correlation studies between IL-23 and the model establishment of lesions of psoriasis.There are less reported about the relation between the level of IL-23 in peripheral blood of the psoriatic patients and the psoriasis. IL-27 is constituted by EBI3 and P28. EBI3 and IL-12 are isogeny and p28 and p35 of IL-12 are isogeny. IL-27 is produced mainly by antigen presenting cells (monocytes, macrophages, dendritic cells) and is highest in monocyte-derived dendritic cells , activated monocytes and activated macrophages. It has not been reported the association study of the levels of IL-27 in the peripheral blood and the severity of the disease.The study is to explore the pathogenesis of psoriasis and provide new ideas in the prevention and treatment of psoriasis by comparing the levels of IL-23 and IL-27 in the peripheral serum of patients with psoriasis and the control group.Methods: The case group involved 29 psoriasis vulgaris patients who were conformed to the diagonsis standard of psoriasis vulgaris and selected in out-patient service in the second hospital of Heibei Medical University from September, 2010 to December, 2010.We had excluded the patients with autoimmunity diseases such as erythema lupus, pemphigus dermatomyositis, rheumatic arthritis and other severe Systemic diseases. The patients had not taken drugs that could affect immune function, such as corticosteroids and immuno suppressive agent and so on in three months. We divided all the case to two groups, one was progressive stage group which had 21examples,the other was stationary stage group which had 8 examples. The control group involved eleven healthy persons without immunological diseases and family history who are staff of the second hospital of Heibei MedicalUniversity. There are no significant differences in sex and age compared with the case group by the T test. By the same staff case group patients to PASI grading,the PASI score is used to the case patients and 2ml venous blood is collected from the case group and the control group. The levels of IL-27 and IL-23 in serum were measured by enzyme-linked immuno sorbent assay after separating the serum. The data were analyzed by SPSS17.0 software. Becease measurement of IL - 23 data meet normality and equal variance,so the levels of IL-23 are statistically by completely random design mean comparison between two groups that are randomly divided a group of patients with psoriasis the adanced group and dormant period group IL - 23 expression level meet normality and equal variance,so the data also can use the same statistic method . The case group and normal controls measured data of IL - 27 are not normality also the IL-27 statistically by non-parametric test. P <0.05 was statistically significance. If the case group and normal controls the results have significant difference the expression of IL-23 and IL-27 is made by The two variables Pearson correlation analysis between the expression of IL-23,IL-27 and relevant level by Pasi score. P <0.05 has the statistical significance.Results:1 The levels of IL-23 in the serum of the patients of psoriasis vulgaris were significantly higher than that of the control group (t=13.657,p<0.05),the difference was statistically significant,the progression stage group and the stationary stage group have no statistically significant (t=0.627,P>0.05). The level of IL-23 in the case group has the positive correlation with the clinical PASI score through the linear correlation analysis(r=0.629, p<0.01)2 The levels of IL-27 in the serum of patients with psoriasis vulgaris in the case group and control group are not according with the normal distribution. So we are using non-parametric test and the two groups had no significance (U = 128.000,P=0.336,P>0.05).Conclusion:1 The expression level of IL-23 in the serum of psoriasis vulgaris was higher than that in the normal control, explained IL-23 participated in the process of psoriasis vulgaris, Shown that high expression of IL-23 is one of psoriasis patients with immuneabnormalities.It is confirmed the onset of psoriasis relevant with T cell immune imbalances.2 The expression of IL-23 in progression stag group and in the stationary stage group have no statistically significant in psoriasis vulgaris serum. Explained the IL - 23 expression level and psoriasis progress was not specific relationship, was not as reaction disease progress indicators.3 The expression level of IL-23 in the serum of psoriatic patients and the corresponding PASI score has line correlation , and for positive correlation, explain serum the expression level of IL– 23 can reflect disease severity.4 It is no correlativity with the severity of the disease from the expression level of IL-27 in the serum of psoriatic patients. The expression that IL - 27 level and psoriasis vulgaris pathogenesis of no specific relevance and Pending further studys.
