Retrospective Analysis Of 131 Inpatients With Ulcerative Colitis

ObjectiveTo evaluate the clinical characteristics,diagnosis and treatment status of in-patients with ulcerative colitis (UC) and guide the clinical therapy.MethodsInpatients with UC during the period from January 2003 to December 2010 were registered and their clinical profiles were analyzed, such as:general state, clinical manifestations, endoscope and pathological features, treatment and prong-osis.ResultsDuring the study period,131 inpatients with UC (77 male,54 female, male-female ratio 1.43:1,median age of 43 years) was identified, of which 29 cases (22.1%) were at the age range of 30-39 years and 26 cases (19.8%) were at the age range of 40~49 years. The course was range from 1 week to 30 years, with a average duration of 2.2 years.2 patients (1.5%) had a clear family history. Most of the hospitalized UC patients (124 cases,94.7%) were in the active stage, only 7 patience (5.3%) in remission. In all cases, the chronic relapsing type of UC was the most common (43.5%,57 cases), followed by the initial onset type (35.9%,47 cases), chronic continuous type (19.8%,26 cases), and fulminant type (0.8%,1 cases). Most of the inpatients with UC were grade as mild (43.5%) and moderate (47.3%), only 12 as severe. The most common gastrointestinal sym-ptorns and signs were diarrhea (82.4%,108 cases), bloody stool (55.7%,73 cases), mucus stool (52.7%,69 cases), abdominal pain 1 (48.9%,64 cases) and abdominal tenderness(55.3%,73 cases).Systemic symptoms were emaciation (26.7%,35 cases).,fever(19.8%,26 cases),debilitation (18.3%,24 cases),anorexia (14.5%,19 cases).About 6.2%of UC patients had complication(intestinal bleeding 3.1%,intestinal perforation 0.8%,incomplete intestinal obstruction 0.8%,colon cancer 1.5%), and 3.9% of UC patients had extra intestinal manifestations ( arthritis 2.3%,uveitis 0.8%,oral ulcers and skin lesions0.8%).By colonoscopy, the change in 97% patients was continuous and diffuse distribution. However,2 patie-nts (1.5%) had skip lesions, only 2 patients (1.5%) were limited to rectosigmoid. 36% patients were limited to the rectum and rectosigmoid, and 24.4% patients had lesions in the left-half colon. The predominant forms of endoscopic manifes-tations were hyperemia and edema (83.2%), followed by erosion or ulceration (81.7%).Mucosal infiltrated by inflammatory cell (including 82.9% patience) was the main pathological manifestations of UC,followed by erosion or ulcerati-on(44.8%).Of the 131 patients, except for 3 patients(2.3%) treated by surgery and 1 patient(0.8%) discharged automatically, the remaining patients were treated with simple medical therapy. The main stay for UC treatment was still 5-amino salicylic acid drugs (5-ASA)(42.7%) and 5-amino salicylic acid drugs combined with glucocorticoids (41.2%), only 2.4% of the patients received immunodepress-ant.Of the 127 patients with simple medical therapy,10 patients(7.9%) were com-plete remission, and 115 patients (90.6%) were significantly improved, only 2 patients (1.6%) were invalid,1 patient died.Conclusion1. Inpatients with UC is common at the ages of 30~49 years, ten years later than abroad.2. The most inpatients with UC is in active stage, mostly with mild to mod-erate severity. The major types of UC are first onset and chronic relapsing.3. The main symptoms of inpatients with UC are diarrhea, bloody stool, mucus stool and varying degrees of abdominal pain, with rare extraintestinal mani-festations and complications. By colonoscopy, the lesions in most patients are continuous and diffuse distribution, mostly limited to the rectum and rectosigmoid and the left-half colon.4. Simple medical treatment is effective, patients needing surgical treatment,cancerization and cases of death are rare, which are lower than abroad。...
