Background:Cervical carcinoma is one of the most common gynecologic malignant tumor and it poses a severe threat to women's life. Some data shows that there are about 13.15 million new cases in our country. Although early diagnosis and treatment of cervical carcinoma has reduced the death rate greatly in recent years,but there are still five million patients died of cervical carcinoma's invasion and metastasis. Invasion and metastasis are biological property of malignant tumor and both of them are extremely complicated pathological process which involved multiple factors. First inhibiting tumor metastasis gene's inactivation makes the tumor cells leave from the primary lesion. Second, the tumor metastasis gene causes the extracellular matrix's degradation.Finally,the tumor cells diffuse from the defection and metastases through blood-vessel and lymphatic channel. How to effectively control invasion and metastasis of cervical carcinoma has became the chiefly task of gynaecology and obstetrics's doctors,because invasion and metastasis are not only the mainly factors that effect the tumor's therapy and recurrence but also the main reason of the patient's death. KAI1 genes are new kind of tumor suppressor genes founded recently, they restrain tumor's metastasis through regulating tumor cell adhesion, inhibiting tumor cell movements and restraining tumor cell of the metastasis lesion's proliferation. Some studies have identified that KAI1 genes have a close relationship with the malignant tumor's invasion and metastasis. MMP-2 gene is one of the members of the matrix metalloprotein's family.It leads to the tumor's invasion and metastasis through degradation the extracellular matrix and membrane and invasion along the missing membrane. ObjectiveTo explore the KAI1 and MMP-2 gene's function and importance in cervical carcinoma's occurrence, development and transfer by researching their expressions in normal cervical tissue, neoplasm of cervical intraepithelial and cervical squamous cell carcinoma and analyzing the relationship of different clinical pathologic characteristics between KAI1 and MMP-2.Methods:Randomly selected surgical resection specimens from January 2009 to October 2010 from daqing quannan hospital.the all specimens were not preoperative chemotherapy and radiotherapy, Immunohistochemical staining reagent positive film box known as a positive control all the slices were identified by two pathology specialists blinded. Immunohistochemical method of the organizations staining to detect the expression of KAI1 and MMP-2. Statistical analysis using SPSS17.0 software. Significance test usingχ2 test, both using spearman rank correlation test correlation analysis。Results:1, KAI1 was mainly expressed in the membrane or cytoplasm carcinoma tissues.In normal cervical epithelial tissue, CINⅠ~Ⅲand cervical squamous cell carcinoma the positive rates of KAI1 were100%,87.5%,73.3%,63.1%, 19.4%。Compared with normal uterine cervix and cervical cancer group was statistically significant, (P<0.05). Compared with Cervical intraepithelial neoplasia and cervical cancer group was statistically significant, (P<0.05). Compared with normal uterine cervix and cervical CIN group was not statistically significant (P> 0.05). Compared with Cervical CINⅠ,CINⅡ,CINⅢwas no significant。(P>0.05). Compared with cervical lymph node metastasis group and non-lymph node metastasis was statistically significant (P<0.05), Compared with well and moderately differentiated and poorly differentiated cervical squamous carcinoma was statistically significant (P<0.05), expression of KAI1 was related to lymph node metastasis and grade of differentiation.there was not was related to tissue infiltration, clinical stage, age, tumor size. (P>0.05).2, MMP-2 positive particles is tan-yellow, mainly in the cell plasma expression, in normal cervical tissue and CINⅠ,ⅡandⅢand cervical squamous cell carcinoma tissues in the pathological specimens, MMP-2 positive expression rate are increasing trend. for 20%,25%,33.3%,47.1%, 75%. Compared with normal cervical tissue and cervical squamous cell carcinoma tissues group was statistically significant, (P<0.05). Compared with Cervical intraepithelial neoplasia and cervical cancer group was statistically significant, (P<0.05). Compared with normal uterine cervix and cervical CIN group was not statistically significant (P>0.05) Compared with Cervical CINⅠ,CINⅡ,CINⅢwas no significant。(P>0.05). Compared with cervical lymph node metastasis group and non-lymph node metastasis was statistically significant (P<0.05), Compared with well and moderately differentiated and poorly differentiated cervical squamous carcinoma was statistically significant (P<0.05), expression of MMP-2 was related to lymph node metastasis and grade of differentiation, but There was not was related to tissue infiltration, clinical stage, age, tumor size (P>0.05).3 In 36 cases of cervical squamous cell carcinoma the expression of KAI1 protein expression of MMP-2 protein-negative accounted for 12 cases, KAI1 protein negative expression of MMP-2 protein expression were a total of 15 cases, while the positive expression of both, in 7 cases, while were 2 cases of negative expression, showing that in cervical squamous cell carcinoma when the KAI1 protein expression showed low expression of MMP-2 protein in many states, but KAI1 negative expression of protein when the high level of MMP-2 protein expression by Spearman rank correlation analysis showed that both negative related, r=-0.526, P<0.01.Conclusion1.The high expression of KAI1 inhibited the development of cervical squamous cell carcinoma.The low expression of KAI1 may indicate more aggressive and poor prognosis.2. The high expression of MMP-2 promote development of cervical squamous cell carcinoma also indicate the high malignancy and poor prognosis.3. In the cervical squamous cell carcinoma, the expression of KAI1 and MMP-2 was negatively correlated. Prompted both in invasion and metastasis of cervical squamous cell carcinoma present in interactions. Combined detect of KAI1 and MMP-2 may can better predict invasiveness and metastasis and judging prognosis of cervical squamous cell carcinoma. KAI1 and MMP-2 gene is expected to become two other targets for cerical cancer treatment.
