BACKGROUND:APRIL(a proliferation-inducing ligand)is a newcomer of the tumor necrosisfactor (TNF) family, which exist in two forms including cytomembraneAPRIL and souble APRIL.Both can contribute to the proliferation of tumor celllines.APRIL in the tissue of normal expression quantity is very low, but in thetumor cell lines and tumor tissue (colorectal cancer, lung cancer, blood systemtumor, breast cancer, melanoma, bladder cancer, etc.) have a high levelexpression,Early clinical studies have found that the expression of APRIL incervical squamous carcinoma tissues, no matter in mRNA and protein levelswere significantly higher than that of normal cervical tissue and squamousintraepithelial lesion, as the cell proliferation and atypia is aggravating,expression level increased gradually.But there is no research on the associationof the APRIL and cervical cancer staging, grading, infiltration depth and lymphnode metastasis, and also have not seen APRIL in the tissue of carcinoma of thecorrelation between expression and prognosis of cervical cancer. This subject we will study the relationship between the APRIL and cervicalcancer,to research the APRIL protein expression profile in normal or chroniccervical tissue,CIN, cervical cancer in various pathological classification, grade,stage and lymph node metastasis.Using immunohistochemical method to detectAPRIL protein expression, research on the relationship between the APRIL andcervical cancer, analysis the relevance of APRIL and cervical cancerpathological classification, staging, grading and lymph node metastasis.Andapplied survival analysis methods of surgical treatment of cervical cancerpatients were retrospectively analyzed clinical data, the possible factorsaffecting the prognosis of single factor and multiple factors analysis, analysis ofAPRIL in the tissue of carcinoma of the correlation of the expression andprognosis of cervical cancer and cervical cancer prognosis prediction systemwas built and used to guide postoperative adjuvant therapy.Clinical studies have found that the expression of the APRIL in cervicalsquamous carcinoma tissues, with the cell proliferation and atypia aggravating,the expression level gradually increased.According to the above results, weassumed that the expression of APRIL with cervical cancer classification, grade,stage and lymph node metastasis increases.In order to verify the aboveassumptions, we detected the sAPRIL protein in77cases of patients withcervical cancer pathological, and10cases of normal or cervicitis tissues,15cases of CIN group comparison, analysis the expression of sAPRIL protein.Reanalysis sAPRIL protein expression in the cancer tissue and the link betweenthe prognosis of cervical cancer. OBJECKT:Research the expression of the APRIL protein in cervical tissue and therelationship with cervical cancer classification, grade, stage,lymph nodemetastasis.METHODS:Immunohistochemical Envision method is used to test the APRIL proteinexpression levels in the organization of77cases of cervical cancer,30cases ofnormal cervical tissue or chronic cervicitis tissues and15cases of CIN.RESULTS:1. Compared the APRIL protein expression among the inflammatory ornormal cervical tissue, CIN tissue and cervical cancer tissue, the difference wasstatistically significant (p <0.05);2.Comparing APRIL protein expression in different differentiated cervicalcancer tissue, the difference was statistically significant (p <0.05);3. Compared the APRIL protein expression among the Cervical squamouscell carcinomas, gland scale cancer, gland cancer tissues, there was nostatistically significant difference (p>0.05);4. APRIL protein expression in different stages of cervical cancer tissue,there was no statistically significant difference(p>0.05);5. Cervical cancer patients with pelvic lymph node metastasis and no pelviclymph node metastasis in cervical cancer tissue APRIL protein expression, therewas no statistically significant difference(p>0.05).CONCLUTIONS:The expression of APRIL protein in normal or chronic cervicitis tissues, CIN tissues and cervical cancer tissues, showed a trend of increasing; APRILprotein expression in cervical cancer tissue is connected with cervical cancertissue differentiation, has nothing to do with cervical cancer staging,clinicalpathology classification and presence of pelvic lymph node metastasis related.II. APRIL protein expression in cervical cancer tissue associatedwith the prognosis of cervical cancerOBJECKT:Analyze the APRIL protein expression in cervical cancer tissue associatedwith the prognosis of cervical cancer,Analysis of surgical treatment of cervicalcancer patients prognosis factors at the same time, used to guide postoperativeadjuvant therapy.METHODS:Surgical treatment of59cases of cervical cancer patients in part one wereretrospectively analyzed the clinical pathological data, characteristic factors ofsix not repeat statistical analysis for single factor and multiple factors.RESULTS:59patients5-year survival rate was81.4%, among them APRIL protein (-)is71.4%, APRIL protein (+)82.7%; IB period was83.3%, type IIA stage was72.7%.APRIL protein (-) and protein (+) group there was no statisticallysignificant difference of overall survival curves (P>0.05). Single factor survivalanalysis it is concluded that the factors related to the prognosis of patients withcervical cancer: pathological type, histological differentiation, lymph nodemetastasis (P <0.05). Multi-factor survival analysis showed that pathological classification,histological differentiation and lymph node metastasis are riskfactors affecting the prognosis of patients with cervical cancer (P <0.05).CONCLUTIONS:APRIL protein positive expression in cervical cancer tissue hasnothing to do with the prognosis. Histopathological classification, pathologicaltype,lymph node metastasis is the IB, type IIA period postoperatively inpatients with cervical cancer related factors affecting the prognosis, earlycervical cancer according to the factors influencing the prognosis of patientswith postoperative adjuvant therapy.
