Backgroud: The incidence of hypothyroidism is not easy to be found, Some patients have little or no symptoms and signs of hypothyroidism corresponding in the early period, and the disease's course is very long. The didisease can affect the cardiovascular system, digestive system, nervous system, blood systems, bones and joints and muscles over the whole body system, clinical manifestations complex and diverse .Because a lot of patients lack of specific symptoms and signs, so very easy to misdiagnosis and delayed treatment, often cause unnecessary suffering to patients. Recent years, scholars have found that there will be some changes with creatine kinase and other muscle enzymes in hypothyroidism patients. What kind of hypothyroidism and muscle enzymes correlation? What effect about hypothyroidism course on muscle enzyme? What kind of dynamic changes with muscle enzymes after hormone replacement therapy? These problems become the focus of our attention. Concluded that can help improve clinical understanding the changes of muscle enzymes caused by hypothyroidism and improve the diagnostic rate of hypothyroidism.Objective: To observed the kind of correlation between hypothyroidism and muscle enzymes. To discussion the effect of different degrees of hypothyroidism on the muscle enzymes and observed the dynamic changes in muscle enzymes after hormone replacement therapy in patients with hypothyroidism.Metheds: Collected 68 cases of patients with hypothyroidism ,21 patients with subclinical hypothyroidism, 15 patients with moderate hypothyroidism, 32 cases with severe hypothyroidism,and 39 cases of healthy.To testing the levels of serum free triiodothyronine (FT3), free thyroxine (FT4), thyroid stimulating hormone (TSH), creatine kinase (CK), creatine kinase-MB (CK-MB), lactate dehydrogenase (LDH) andα-hydroxybutyrate dehydrogenase isoenzyme (HBD), aspartate aminotransferase (AST), troponin (anTL) in the first visit, one month after hormone replacement therapy and two month after hormone replacement therapy . Result:①Patients with primary hypothyroidism CK, CK-MB, LDH, HBD, AST were higher than the control group , The magnitude of increase in a variety of muscle enzymes are differet and the most obvious increase in CK.②The enzymes are related to FT3 and FT4 were negatively correlated, with TSH was positively correlated. Between CK and thyroid function have the best correlation.③Different degrees of hypothyroidism had different effects on muscle enzyme. Severe hypothyroidism group have most significant change in muscle enzymes.④After hormone replacement therapy , the levels of muscle enzyme gradually returned to the control group in patients with hypothyroidism, all the muscle enzymes returned to normal after 4 to 6 weeks.Conclusion: Hypothyroidism can cause increased muscle enzymes.The more obvious increase in muscle enzymes with the increased degree of Hypothyroidism. With the hormone replacement therapy, the resumption of muscle enzymes after hypothyroidism condition improved gradually, muscle enzymes returned to normal before the TSH.
