Background The incidence of primary hypothyroidism is high,the clinical work found that the like for women and the elderly.Research shows that primary clinical hypothyroidism can elevate levels in low density lipoprotein cholesterol(LDL-C),total cholesterol(TC),glycerin three greases(TG),high density lipoprotein cholesterol(HDL-C)levels decreased and blood lipid disorders.Due to the increase of LDL,TC,TG levels of heart disease risk factors of atherosclerosis,so the study of primary hypothyroidism and blood lipid levels of patients has important significance to explore its relationship with lipid subclass.Objective To study the changes of serum lipoprotein cholesterol subclass in patients with primary hypothyroidismMethods The subjects were diagnosed with primary hypothyroidism at the First Affiliated Hospital of Medical University Of Anhui,38 cases of primary hypothyroidism were divided into clinical hypothyroidism group(hypothyroidism group,20 cases)and subclinical hypothyroidism group(subclinical hypothyroidism group,18 cases),Another 20 healthy persons(control group)were selected as control group.5 ml of fasting venous blood was extracted,the serum was separated and preserved at-80? in a freezer.Configuration of density liquid and ultracentrifugation under different background densities was done.Cholesterol oxidase method was used to determine the serum cholesterol,levels of small and low density lipoprotein(sd LDL),high density lipoprotein 2(HDL2),and high density lipoprotein 3(HDL3).At the same time,levels of serum triiodothyronine(T3),thyroxine(T4)and thyroid stimulating hormone(TSH)were checked.Statistical analysis of the correlation between the thyroid relevant hormonementioned above and serum lipoprotein cholesterol subclass was done.Results(1)Data between groups of gender and age no significant difference,body mass index(bodymassindex,BMI)showed no significant difference(P> 0.05),In patients with subclinical hypothyroidism group and hypothyroidism group,the total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL)level is higher than that in the normal control group,high density lipoprotein(HDL)level is lower than the control group,a statistically significant difference(P<0.05).(2)The determination of cholesterol by ultracentrifugation with COD-PAP method,subclinical hypothyroidism group and hypothyroidism group sd LDL-C was higher than the control group,a statistically significant difference(P<0.05);hypothyroidism and subclinical hypothyroidism group serum HDL2-C lower than the control group,a statistically significant difference(P<0.05);HDL3-C had no statistical significance(P>0.05);subclinical hypothyroidism group and clinical hypothyroidism group sd LDL-C,HDL2-C and HDL3-C had no statistical significance(P>0.05).(3)The determination of cholesterol by ELISA kit,sd LDL levels increased in hypothyroidism group and subclinical hypothyroidism patients,compared with the control group between the groups was statistically significant(P<0.05);the levels of HDL2 in patients elevated with subclinical hypothyroidism group and hypothyroidism group,compared with the control group between the groups was statistically significant(P<0.05);there was no significant difference between the HDL3 group(P>0.05);hypothyroidism group and subclinical hypothyroidism group were not statistically significant.(4)The primary hypothyroidism patients,determination of cholesterol results obtained by ultracentrifugation with COD-PAP method,the level of sd LDL-C was positively correlated with TSH and negatively correlated with T4,and T3 had no significant correlation with T3;HDL2-C and T4 were positively correlated;no significant correlation between HDL3-C and TSH,T3,T4.(5)In primary hypothyroidism patients,determination of cholesterol by ELISA kit,sd LDL-C was positively associated with TSH,no significant correlation between T3 and T4;HDL2-C was negatively correlated with TSH,T3,and T4 had no significant correlation,no significant correlation between HDL3-C and TSH,T3,and negatively correlated with T4.Conclusion(1)The changes of serum lipids and lipoprotein cholesterol in patients with hypothyroidism and their correlation with thyroid function.The increase of serum TG,TC and LDL in patients with hypothyroidism is a common phenotype of dyslipidemia.(2)Hypothyroidism and subclinical hypothyroidism patients with elevated serum sd LDL-C levels and decreased HDL2-C,HDL3-C was not statistically significant;hypothyroidism and subclinical hypothyroidism group compared with sd LDL-C,HDL2-C,HDL3-C was not statistically significant.Elevated sd LDL-C levels and decreased HDL2-C is one of the manifestations of dyslipidemia in patients with primary hypothyroidism,and may be an important cause of dyslipidemia in patients with hypothyroidism.(3)Correlation analysis sd LDL-C level was positively correlated with TSH,HDL2-C and HDL2-C were negatively correlated with TSH,HDL3-C and TSH,T3,T4 were not significantly correlated.The results obtained by ultracentrifugation were not coincident with the results of ELISA kit,but the overall trend was consistent.
