Clinical Research On TCM Syndromes Of Essential Hypertension In The Elderly

Objectives:The Objective of this study is to discuss the distribution regularity of TCM syndrome patterns of the essential hypertension in the elderly and the correlation between syndrome patterns and some objective examinations. It is expected to provide some evidences of differentiation of syndromes and to improve the level of differentiation of syndromes and treatment in the essential hypertension in the elderly.Methods:Through clinical research, we collect patients' basic instances, clinical symptom, information from four diagnostic methods of TCM,and diagnosed TCM syndromes on basis of syndrome diagnose standard. Through spastics, we studied the relations among regularity of TCM syndromes and somefactors including sex, age, medical history of EH,hypertension family history,high-salt diet, alcohol, impatience, BMI and low physical activity.Results:1.In this investigation, according to the frequency, the sequence of main syndromes were syndrome of yin deficiency and yang hyperactivity (34.6%), syndrome of deficiency of both yin and yang (18.9%), syndrome of excessive phlegm-dampness (17.6%), syndrome of static blood blocking collaterals (12.8%), syndrome of exuberance of liver fire (10.9%), syndrome of deficiency of both qi and yin (4.2%). Male and female patients in the TCM syndrome distribution was not significantly different (χ2=3.763, P= 0.584) 2. The TCM syndrome distribution of different age were significantly different (χ2=45.30, P<0.05)。The pattern of syndrome of excessive phlegm dampness and syndrome of exuberance of liver fire decrease with the increasing of age,but syndrome of deficiency of both yin and yang and syndrome of static blood blocking collaterals permit an increase in the proportion of old age. The average duration between syndrome of yin deficiency and yang hyperactivity,syndrome of deficiency of both yin and yang, syndrome of exuberance of liver fire, syndrome of static blood blocking collaterals and syndrome of excessive phlegm dampness is significant different (F=25.546,P<0.05).The average duration of syndrome of deficiency of both yin and yang is the longest, shortest syndrome of exuberance of liver fire.3.There is a significant statistical difference in Classification of BP between different syndrome patterns (χ2=23.45, P<0.05). syndrome of exuberance of liver fire and syndrome of excessive phlegm dampness are major patterns in grade land grade 2 of EH, syndrome of yin deficiency and yang hyperactivity mainly grade 2, syndrome of deficiency of both yin and yang and syndrome of static blood blocking collaterals mainly grade 3.4. The increase of SBP is significant in the pattern of syndrome of deficiency of both yin and yang and syndrome of static blood blocking collaterals. The increase of DBP is significant in the pattern of syndrome of exuberance of liver fire and syndrome of excessive phlegm dampness.5. The risk factors Logistic regression analysis showed that:syndrome of yin deficiency and yang hyperactivity and high-salt diet, duration of 3 years period(10-years) showed a positive correlation. Syndrome of static blood blocking collaterals and lack of exercise, duration of 4 years period (15 years) showed positive correlation. Syndrome of deficiency of both yin and yang with the course of two years of paragraph 5 (≥20 years) showed positive correlation. Syndrome of excessive phlegm dampness and alcohol, BMI increased positive correlation. Syndrome of exuberance of liver fire and irritable personality, age 1 (<70 years old) showed a positive correlation.Conclusion:The sequence of main TCM syndromes of the essential hypertension in the elderly were syndrome of yin deficiency and yang hyperactivity, syndrome of deficiency of both yin and yang, syndrome of excessive phlegm-dampness, syndrome of static blood blocking collaterals, syndrome of exuberance of liver fire. The TCM syndrome distribution had intimate relation with age and course of the disease, excess syndrome dominated in the early days, with the course of the disease progress, syndrome of intermingled deficiency and excess gradually increased, blood stasis factors also became clear during the long illness. High-salt diets, alcohol, lack of exercise, obesity, low physical activity, impatience have an impact on the TCM syndrome distribution of the essential hypertension in the elderly.
