| Objective To investigate the clinical value of Explore totally extraperitoneal hernia repair (TEP) in the treatment of indirect inguinal hernia and provide scientific evidence.Methods Review part of the surgical treatment of inguinal hernia patients in the Minimally Invasive Center, the First and the Ninth Affiliated Hospital of Guangxi Medical University from June 2004 to June 2009,respectively, in the "Totally extraperitoneal hernia repair" ,"Hernia ring filling tension-free hernia repair"," Traditional surgery "in the three groups of surgical patients were randomly selected from 76,73,61, then follow up and analyze the items include Incision length, operative time, blood loss, postoperative pain index, after other complications, length of stay, relapse rate,complications.Results 1. Totally extraperitoneal hernia repair group operative time with the hernia ring filling tension-free hernia repair group of patients with no significant other, no statistically significant (P >0.05), between the operation time shorter than those of conventional surgery group , there are statistically significant (P <0.05); The incision length totally extraperitoneal hernia repair group shorter than the hernia ring filling tension-free hernia repair group and the conventional surgery group, there are statistically significant (P <0.05); In the blood loss is completely extraperitoneal hernia repair group less than that of the hernia ring filling tension-free hernia repair group and the conventional surgery group, it is statistically significant (P <0.05). 2. Totally extraperitoneal laparoscopic hernia repair postoperative pain index less than that of the hernia ring filling tension-free hernia repair group and the conventional surgery group, and it is statistically significant (P <0.05); In the postoperative complications (urinary retention, scrotal swelling, wound hematoma and infection) of the totally extraperitoneal hernia repair group than that of the conventional surgery group, there are statistically significant (P <0.05), but it is no difference singnificant between the complete extraperitoneal hernia repair group and the hernia ring filling tension-free hernia repair (P >0.05) . 3. The time of stay at the hospital of the completely extraperitoneal hernia repair group is shorter than that of the other two groups, there are statistical significance difference(P <0.05); The recurrence rate of the completely extraperitoneal hernia repair group is no difference than that of the hernia ring filling tension-free hernia (P >0.05), but the recurrence rate of two groups are lower than that of the conventional surgery group, there are statistically significant difference (P <0.05).Conclusion 1. Totally extraperitoneal laparoscopic hernia repair in the treatment of adult indirect inguinal hernia can shorten the length of surgical incision, reduce blood loss, shorter hospitalization time, relieve pain compared with that of the other two groups. 2. Totally extraperitoneal laparoscopic hernia repair in the treatment of adult indirect inguinal hernia at the operative time, postoperative recurrence rate compared with that of the hernia ring filling tension-free hernia repair group was no significant difference, but both is better than that of the conventional surgery group, the postoperative complications are completely extraperitoneal laparoscopic hernia repair group than conventional surgery group, with the hernia ring filling tension-free hernia repair group was no significant difference. The totally extraperitoneal laparoscopic hernia repair is a safe and prognosis beautiful surgical method , so it can be widely used in clinic. |
