Study On Depression Among Health Professionals And Its Influcing Factors In County Public Hospitals In Shandong Province

Background:Mental health problems of health professionals have been increasingly concerned in recent years. Medical work is a high stress job and health professionals are prone to depression feelings or symptoms because of tightness, being caused by heavy treatment and nursing work, social competition, pressure in life, different hospital manegement and individual character etal. Currently, according to statistics, doctors' medical risk of depression are from 25% to 30%, more than 4 times general population. Doctors suffering from depression are in a higher risk of suicide, male doctors being 3.4 times and female doctos being more than 4 times compared with the general population. Depression not only affects the physical and mental health, also affects the attitude of health professionals and the medical quality, leading to a hidden security trouble. So, depression of health professionals cann't be ignored.Objectives:The study aims to understand the depression status among health professionals in county public hospitals in Shandong Povince and analyze its influencing factors, so as to provide recommendation for early prevention and treatment about health professionals'depression, to improve their work performance and quality of medical services. Methods:In this study, a multi-stage stratified cluster random sampling method was used. The study interviewers were health professionals in county pubilc hospitals in Weihai, Dongying, Heze, in Shandong province. Self-made questionnaires were used in this survey and they were finished by health professionals under the guidance of investigators uniformly trained. 3200 questionnaries were distributed and 2903 copies were collected, the effetive rate is 90.7%. ACESS software was used to establish the database and SPSS 16.0 and Lisrel 8.70 were used to statistical analysis. Data analysis methods are:(1)To descriptive the general situation: mean and standard deviation were used in the numeric data or nearly numeric data; rate and proportion were used in the categorical data. (2) X2 test was used in the univariate analysis of the prevalence rate of health professionals, Spearman rank correlation coefficient was used in the correlation analysis. (3) Binary Logistic Regression was used to explore the influencing factors in the multivariate analysis.Main Results:(1) The positive rate of depression in this survey is 36.3%. Among it, the mild depression rate is 13.7%, the moderate is 17.3%, the severe is 5.2%. (2) The univariate and multivariate analysis showed that anxious and depressive feelings, health index, the number of physical activity per week, sleep quality, patients with or without excess behaviors to health professionals, how much confidence of patients in health professionals, self-evaluation on vocational environment, re-choice of occupation, subjetcive support and utilization of support were main influencing fators of the positive rate of health professionals. Among this,①The positive rate of depression among health professionals without anxiety or depressive feelings is lower than those with extreme anxiety or depressive feelings;②The lower health index is, the more depression occur;③The positive rate of health professinals who exercise more times per week(more than six times) is lower than those who never exercise;④The better sleep quality is, the lower positive rate of depression is;⑤The positive rate of depression among health professionals who suffering patients' insulting language is high;⑥Compared to those having a bad assessment on occupational environment, the positive rate of depresion among health professionals having a good assessment on occupational environment is lower;⑦The positive rate of depresion among health professionals who are given a high trust from patients is low;⑧If have a re-choice, the positive rate of depresion among health professionals who choose the current job is low;⑨Health professionals whose subjective support scores are high or support utility are good have a low positive rate of depression.Conclusions and suggestions:(1) The positive rate of depression among health professionals in county public hospitals in Shandong Province is high, so related departments should pay more attention to it. (2) Health status, physical activity, sleep quality, conflict between patients and health professionals, professional recognition, social support are main influcing factors of depression among health professionals. (3) According to the results of this survey, the following recommendations were made for reference: for the hosptal:①inproving the system and management;②paying more attention to the mental health and establishing prevent and teatment system;③enriching culture and leisure activities;④creating harmonious and humane-care environment. For the individuals:①learning positive coping, increasing phycological competence;②raising the personal accomplishment, initiativing to impove physician-patient relationship;③having a correct awareness of the occupation value;④cultivating personal hobbies consciously.
