The Clinical Observation On Retention Enema Of TCM For The Treatment Of Active Ulcerative Colitis

Purpose:Ulcerative colitis is a kind of chronic non-specific inflammatory bowel disease, mainly involving the rectum and colon mucosa and submucosa. Clinical manifestations of the disease include diarrhea, and mucopurulent bloody stool, with abdominal pain, tenesmus, and different degrees of systemic symptoms. UC is mostly chronic onset, breaks out continuously or repeatedly, and the pathogenetic condition is differ. In recent years, as people living standard rise, especially change of diet structure, living habit and environment, the incidence of ulcerative colitis in China increased yearly. The etiology has not been known clearly, the western medicine treatment has defects, such as high price, easy recurrence after drug withdrawal and systemic reaction. So the treatment of traditional Chinese medicine’s advantage gradually emerged. In clinical, external therapies of TCM have the advantages, such as simple and easy to do, few side effects and so on. This topic is to observe the retention enema of TCM combined with western medicine for the treatment of ulcerative colitis clinical efficacy, to improve the effect of the treatment of the disease, to increase the clinical experience, and to provide a new concept for diagnosis and treatment.Method:Selected 60 cases of spleen deficient ulcerative colitis from 2014 to 2016 in Taizhou TCM hospital outpatient and hospitalization, randomly divided 60 cases into treatment group of western medicine+retention enema of TCM (30 cases) and control group of single western medicine (30 cases). The treatment group with Mesalazine enter-coated tablets(sunflower pharmaceutical H19980148 approved by 0.25 g per piece) 1g qid po, supplemented by retention enema qn, the control group with single Mesalazine enter-coated tablets 1g qid po,10 days is one period of treatment,2 day intervals between the two period of treatment, carried for 2 periods continuously. Mayo score, colonoscopy mucosal lesions, clinical and TCM symptom score and related laboratory characteristics were observed before and after therapy.Results:Observing the two groups with before and after treatment, on the Mayo score, the treatment group was superior to the control group (P< 0.05); in inflammatory marks reacting the activity of ulcerative colitis, the degree of drops of erythrocyte sedimentation rate and C-reactive protein of the treatment group was superior to the control group (both P< 0.05); on colonoscopy improvement, the treatment group and control group effectiveness were 96.7%,73.3% respectively, the control group was lower than the treatment group (P< 0.05); on the therapeutic effect of western medicine, the efficiency of treatment group was 96.7%, the control group effectiveness was 73.3%, on the TCM evaluation curative effect, the treatment group response rate was 96.7% and 70.0% in the control group, treatment group was superior to control group (both P< 0.05).Conclusion:On the Mayo score, colonoscopy improvement, inflammatory markers and clinical effects, the treatment group was totally superior to the control group. This proves that retention enema of TCM combined with western medicine for the treatment of spleen deficient ulcerative colitis can significantly improve the main symptoms, repair intestinal ulcer surface and reduce the systemic inflammatory response in patients. As a simple and feasible method, it has better clinical efficacy and less side effects.
