Analysis on the Relationship between Death Causes and Metabolic Syndrome in a Group of Elderly Population during 13 Years'Follow-UpObjective To analysis the relationship between metabolic syndrome and the causes of death in the elderly population who were followed up for 13 years. Methods Retrospective analysis was performed on elderly population who received medical examination every year from 1996 to 2009, to evaluate the prevalence of metabolic syndrome and mortality, death causes distribution during 13 years. The influence of metabolic syndrome on mortality was also investigated. Result 576 out of 2160 cases died during these 13 years (26.67%). The major death causes were malignant tumor, respiratory system and cardiovascular system diseases, the mortalities of which were 36.98%,31.42% and 18.23% respectively. The prevalence of diabetes, hypertension, over-weight, HTG and Low HDL-C were 40.28%,72.18%,46.99%,47.55% and 7.22% respectively. Multi-factor analysis showed that the age (OR=1.17,95%CI=1.14~1.19) and hypertension (OR=1.39, 95%CI=1.08~1.78) were independent risk factors for death. The mortality of hypertension group (29.31%) was higher than non-hypertension, and the mortality of obesity, glucose and lipid metabolic disorder groups were identical to corresponding groups. The prevalence of cardiocerebral vascular diseases increased with the aggravation of metabolic syndrome components, while the mortality did not change significantly. Major death causes were still malignant tumor, and respiratory system diseases. Conclusion The major death causes of this elderly population are malignant tumor, respiratory system and cardiovascular system diseases. Aging and hypertension, but not metabolic syndrome components aggravations increase the death risk of this population. Survey on Comprehensive Intervention of Risk Factors in Metabolic Syndrome in Elderly MaleObjective To observe metabolic parameters rate and its influence factors, comprehensive intervention especially education and lifestyle modification was given to elderly male patients with metabolic syndrome who have been treated.. Methods Research objects:189 patients with metabolic syndrome by the standard of CDS were chosen from people who had physical examination in 2009. They were divided into intervention group (n=75) and control group (n=114). Research methods: First,189 patients'physical examination data include:birth, height, weight, blood pressure and blood biochemical indexes.75g OGTT was used to screening diabetes. Second,75 patients with poor metabolic parameters control as intervention group were collected history, height, weight, waist and hip circumference, blood pressure, blood biochemical indexes, HbA1C and PWV in September 2009. And above indexes were measured again in December except PWV in order to supervise them and guide treatment. Third,189 patients with metabolic syndrome were measured indexes above at the end of follow-up. Fourth,75 patients were informed to answer metabolic knowledge questionnaires which 56 patients had answered, and 49 patients of them completed more than 50%. Comprehensive interventions include diet control, keep exercise, health education and drug therapy. SPSS 13.0 was used to analyze statistics. Result 1.more than half patients in intervention group have arteriosclerosis in bilateral lower extremities. Every metabolic parameter in intervention group is worse than that in control group, especially FPG, TG, TC, LDL-C, SBP and DBP (P<0.01).2. In intervention group, TC, TG and LDL-C control rate in 2010 is significantly higher than that in 2009 (P<0.01), and also blood uric acid control rate (P<0.05).3. In control group, metabolic parameters rate is not higher in 2010 except LDL-C (P<0.05).4. Compared with lifestyle intervention, drug treatment can lower TC, TG and LDL-C level more effectively.5. In 49 metabolic knowledge questionnaires,31 out of them have more than 60% accuracy. By age,80-89 age group has lowest accuracy.85.11% out of 49 patients believe that poor self-restrained is the main reason of poor metabolic parameters rate, and 48.94% patients believe that ignorance of the harms caused by metabolic syndrome or lack of treatment knowledge when there are changes in daily life is the main reason. Conclusion First, All metabolic parameters rate in this elderly male population is low, especially blood pressure and weight. Second, for elderly people with metabolic syndrome who have been treated by drugs combination, it is difficult to make their weight, blood glucose and blood pressure up to standard. Third, blood TC, TG, LDL-C can be well controlled through intensive lipid-lowering therapy. Fourth, most patients in this group believe that the main reason of poor metabolic parameters rate is poor self-restrained, and almost half of them believe that the main reason is ignorance of the harms caused by metabolic syndrome or lack of treatment knowledge when there are changes in daily life.
