Research On City New-type Rural Cooperative Medical Financing Compensation And Its Influencing

New-type rural cooperative medical system is a heavy sickness based farmer medical mutually system, which is organized, guided and supported by government and farmers take part in voluntarily, funded by the individual, collective and multi-government. It's a new system that covers hundreds of millions of farmers with the purpose of reducing their economic burden and improve their health level, which has an important significance to promote rural economic development and make the society stable. Through the urban new-type rural cooperative medical scheme work and various data and questionnaires, this paper aims to analyze urban new-type funding compensation, find out problems in the new rural cooperative fund-raising compensation, make analysis of factors affecting compensation with the sample of new-type rural cooperative medical system (hereinafter referred to as the new rural cooperative). Based on the research result, methods and procedures suiting to new rural cooperative fund-raising and compensation, actions to improve the level of protection of the policy are proposed, the masses of peasant basic health care needs is solved and the phenomenon of"back into poverty due to illness, lead to povery due to illness"is avoided.Material and methods: use statistical analysis method for the research of new rural cooperative compensation status. Use stratified cluster sampling method to conduct self-surveys and discussion with farmers face to face or interview. Use document summarizing method for fund-raising and compensation factors.Result: 2008 urban people number: 370650, fund-raising amount: RMB 29.652 million, people number receiving compensation: 120683, compensation amount: RMB 24.6754 million. Benefit rate: 32.52%, fund utilization rate: 83.22%, average hospitalization cost: RMB 5195, average compensation cost: RMB 1621, among which 876 for township hospitals, 1177 for district hospitals, 1944 for provincial and municipal hospitals. Compensation rate is 31.20%, among which 54.05% for township, 40.02 for district level hospitals, 28.68% for provincial and municipal hospitals. Capital Flows: compensation provincial and municipal medial institutions takes 76.49%, district-level medical institutions account for 15.38% and rural medical institutions takes 8.13%.Conclusion: through surveys and analysis results, factors affecting new urban co-financing are showed to include the following(1) from the government point of view, policy is instability, enforcement is not strong, rural cooperative medical financing is inefficient, cost is much high, publicity lacks target, effectiveness and scientific management factors.(2) from the collective point of view, difficult and low level of fund-raising are the main factors affecting the farmers participate in new rural cooperative system. Therefore, the rural collective impact can not be ignored in order to development new rural cooperative medical system.(3) factors of farmer's own: because advertising is not in place, farmers lack of understanding of new rural cooperative system, besides, new rural system has its own problems, such as no easy and reasonable way to pay, limited new rural system plan design, funding level can not meet the farmers needs, range of basic medicine and basic treatment needs to be widened, low fund utilization rate, fast average hospital cost increasing, low hospitalization rate and outpatient attendance rate, low level of protection, unreasonable patient flow and fund flow. The negative effect is to reduce the farmer's enthusiasm, leading to the no understanding from farmers to new rural system and no strong will to voluntarily join the system.Therefore , in order to better promote the new rural cooperative development, the following aspects of work must be implemented: (1) to strengthen cooperation in health care financing for the Government as a compensation for the subject of Stare responsibility, including stepping up publicity to raise farmers awareness rate; build a reasonable, simple and effective financing mechanisms for farmers; consolidate the legal basis of cooperative medical compensation to ensure the rigidness of compensation policy; strengthen the Government's financial support to cooperative medical care; improve co-ordination level, reduce risks and make sure of the necessary support systems for rural medical care; effectively improve and enhance the rural medical sector service conditions and service quality, actively promote the rural health service system; (2) increase the compensation efforts for rural medical fund-raising, reduce compensation cost, strengthen the fund supervision, improve the satisfaction rate of farmer participation together, including effectively participating in financing expansion side, encouraging the investors, promoting financing compensation to be medical socialization and achieving the target of minimizing fund-raising operating cost.(3) establish the regulatory mechanisms for new-type rural cooperative medical fund-raising compensation, including restraint mechanisms and implement the principle of good service and solving the problems for people.(4) determine the funding levels and funding ratio for individual contributions, group support, government funding and other aspects. (5) reasonably determine the mode of financing, scientific design the compensation plan for new rural system, expand the compensation scope, increase the fund utilization, improve the benefit from the cooperative medical system and thus guide the participation farmers to get treatment scientific. Rationally use health resources to improve the rural health service capacity, to achieve the integration of rural health services, which is the important factor of building up rural new-type cooperative medical compensatory mechanisms.
