| In recent years, with the development and perfection of the technology of a variety of cesarean section, and the application of anesthesia and various effective antibiotics, the safety of cesarean section has been greatly improved, resulting in the broadening use of cesarean section. Most hospitals report that the rate of cesarean section has reached to some 40% -60% or even as high as 70% -80% or so. If This abnormal situation would not be corrected, it will cause great risks to the health of mother and child.This study is based on the analysis of the 2000-2008 cases in the Maternity Department of our hospital,selecting 200 cases respectively for each year of 1990,1995,2001,2004 and 2008 and making comparison between each year's cesarean indications. The subject of the study is 300 pregnant women (with full-term normal head position, without any obstetric complications, excluding the mentally ill) who produced in our hospital from January 2009 to September 2009.Questionnaires are used to investigate these 300 pregnant women's natural conditions (age, domicile, occupation, ethnicity, economic status, educational level, etc.), then, this thesis will evaluate the relationship between these factors and social factors causing Cesarean Section, and use SAS and SDS Psychological Inventory and Eysenck Personality Questionnaire to do psychological evaluation on the pregnant women who choose cesarean section due to social factors, and also analyze the relevance between individual psychologicalfactors and social factors of cesarean section.The results of the study show that: (1) over the past decade, in our hospital, cesarean section rate is on rise. (2) over the past decade, the proportion of basin factors related to cesarean section in our hospital is annually declining, the percentage of those choosing cesarean section due to social factors is on the top of the first indications for cesarean section. Those who choose cesarean section due to social factors and have no demand of indications accounted for more than half of the cases. The main reasons are: lack of confidence of childbirth, being afraid of vaginal delivery neonatal intelligence and iatrogenic factors. (3)the relationship between the caesarean section and social demographic factors: The affecting factors to the cesarean delivery rate include—pregnant women's age, their education level and their family members'educational level, health insurance forms, occupation, place of residence, economic level, dominator for cesarean section, the number of perinatal health care, knowing about the infants'sex, attending the school for pregnant women, knowledge on maternal cesarean section. (4) the relationship between Cesarean section and pregnant women's personality and psychology: The rate of cesarean section is influenced by anxiety, depression, emotional instability, mental quality, outward-looking character of pregnant women. (5)Logistic regression analysis on cesarean group show that: the cesarean section rate is also influenced by maternal age, economic level and neuroticism, of which neuroticism is most dangerous.Based on the above analyses and studies, this thesis aims to provide a crude reference with a view to clinical practitioners of obstetrics and gynecology. Novelty of this study is the use of SAS and SDS Psychological Inventory and the Eysenck Personality Questionnaire on maternal psychological evaluation and take the pregnant women's psychology and personality into the analysis of the affecting factors to cesarean section. This study also has some limitations. There is not enough deep exploration in the theoretical system and the perspective is not broad enough.At the same time, the origin of pregnant women varies from hospital to hospital , there are different social demographic factors. The subject of this study only come from our hospital which is a three A-level hospital, quite different from the two A-level hospitals and township hospitals, the study has its limitations on the subject.As more and more people are concerned about the means of production, it is worthy of further research in a comprehensive and systematic survey for the cesarean section due to social factors, and further proposed intervention strategies to reduce social factors indication of cesarean section rate to protect pregnant women and their children'safety. |
