Object:The purpose of this study is to explore the pharmaceutical care of pharmacists in Wuhan drug stores by observation and to analyze and summarize the influential factors of the pharmaceutical care. After observation and systematical analysis, suggestions on bettering the service quality of pharmacists in drug store will be given.Data and Methods:Literature review, observation method, DELPHI and statistical method were used in this study. The observation method was used in the non-participation way, and services of the pharmacists in drug stores were surveyed in 60 pharmacies, which were divided into large, medium and small size. In order to avoid the differences of passenger flow which were caused by geographical positions, the similar pharmacies geographical positions were chosen. The pharmacies in central areas were selected in this study. The research of service of every pharmacist began from the opening time in the 3 days, and last 8 hours in every day. The study would be delayed one day when the pharmacist was absent.The data included: (1) Literatures concern the functions of pharmacists in drug store both at home and abroad. (2) The regulations, formal documents and management pattern for pharmacists in drug store. (3) Data of pharmacists in drug stores, including job data and pharmaceutical care data of pharmacists in drug stores in Wuhan. The job data contain the following items: the qualifications and certification owned by the pharmacists, substituted quality controller in charge and the legal persons of the drug enterprises, the hanging of pharmacists'licenses to practice, the wearing of identity cards and the attendance of pharmacists'. The pharmaceutical care data include:; the drug quality management data include the categorizing of drugs, medicine storage and information checking; the medication guidance data of medical services and prescription services; the recommendation of OTC drugs in antipyretic,analgesics and anti-inflammatory.Results and Conclusions:1. The job data is the prerequisite foundation of the pharmaceutical care. The job data showed that 90.89% pharmacists also functioned during their work hours as the quality responsible persons, the rate of Hanyang, Qiaokou, Jiangan and Jianghan was 85.29%,94.71%,91.93% and 91.20% respectively, while 12.71% pharmacists as the business responsible persons, and the rate of the four sections was 4.12%,0.59%,21.08%,20.37% respectively. There were significant differences among pharmacies of different size in the aspect of hanging the pharmacists'certifications to practice, wearing identity cards and holding posts, and the differences were positively correlated with the size of the pharmacies(all P<0.05).2. The rate of reflecting drugs'classification storage was 81.67%, where the rates of the large and medium size pharmacies were both 100%; the rate of information checking was 83.33%, and the rate of pharmacies of all size was above 70%.The medicine guidance was studied by the service length per person per time of total medication guidance and that of prescription drugs'guidance. The two indexes were calculated from the service times, service persons of total medication guidance and that of prescription drugs'guidance. The service length per person per time of total medication guidance of pharmacists in large, medium and small pharmacies was 5.86, 4.42 and 2.24 minutes respectively, and the service length per person per time of prescription drugs'guidance was 3.77, 3.15 and 2.12 minutes respectively .There were significant differences in service length per person per time of total medication guidance and that of prescription drugs'guidance among pharmacists in different sized drug stores(F value were 199.27,47.73 respectively, all P were 0.000 when the Bonfferoni method was used), and the service time correlate positively with the scale of drug stores; Among pharmacists in drug stores of large and medium size, there were significant differences between the two indicators, but not in the small level (t value were 10.002, 6.577 and 1.049 respectively, P value were 0.000, 0.000 and 0.301 respectively).The rate of drug recommendation was 81.90%, and the rate of large, medium and small drugs was 86.40%, 80.70% and 61.60% respectively. There were significant differences among the pharmacies of different size in the recommendation length, and the differences were positively correlated with the size of the pharmacies(χ~2=144.888,P<0.05;the rank correlation coefficient was 0.162, P<0.05).3. The remaining problems include the irrational function of working related conditions; the difficulties in drug quality management; the difference of medicine quality control existed among drug stores of various scales; introducing medication; poor service quality of small drugs'; limited contents of pharmaceutical care; the lacking of criterions in study; the difficulties in combination of pharmaceutical care concepts at home and abroad.4. The factors that place influences on pharmaceutical service provided by druggist in pharmacies included national policies and regulations, department supervision, pharmacist's access and education system, the management pattern and marketing modes of drug stores, the willingness of pharmacists and social cognition.Suggestions:1. Clarifying the important position and service contents of pharmaceutical care.2. Reforming the educational system aiming at training high-quality pharmaceutical personnel3. Establishing effective pharmaceutical care regulations. The drug stores should be equipped with pharmacists according to the specified regulations, set up the advisory area for the pharmacists, establish drug calendar system, record adverse reflect information, express application of service conventions and supervisory telephone.4. Establishing a scientific pharmaceutical care service procedure. In order to effectively meet customized demand, the drug stores should consider seriously the participation, preferences and demand of consumers, and design pharmaceutical care service procedure.5. Implementing effective pharmaceutical care performance evaluation system. The contents of evaluation system should involve quantity and quality of service, customers'satisfaction rate, market share and profitability and the reputation.6.
