Objective: To observe the clinical efficacy of using Gegenqinlian Modified Decoction to treat acute radiation enterocolitis, and approach the correlated mechanisms.Methods: (1)Clinical reseach: Divided 51 patients of damp-heat syndome of radiation enterocolitis into two groups, one group(treatement group) of 26 patients were treated with the Gegenqinlian Modified Decoction, and the other group(control group) of 25 patients were treated with symptomatic treatment of western medicine. Therapeutic effect includes total effective rate, cure rate, disruptions of radiotherapy, and integrate changes of symptoms and signs of diarrhea, abdominal pain, tenesmus, hematochezia, anal pain and loss of appetite. (2) Experimental studies: 28 rats were randomly divided into Gegenqinlian Modified Decoction group(A group), compound glutamine(B group), model control group(C group) and normal comtrol group(D group), and each group had 7 rats. A group, B group, C group were underwent abdominal irradiaton with X-rays at single dose of 9 Gy. 6 days before irradiation, A group were given Gegenqinlian Modified Decoction orally, B group were given compound glutamine orally, C group and D group were given normal sodium. All rats were given NS or medicine once a day for 10 days. Rats were killed 4 days after irradiation. Tissue from The terminal ileum and the middle of rectum organizations are retaken. After 10% formalin-fixed for 3h, paraffin-embedded, biopsy, and HE stain, the morphologic changes of intestinal tissue were observe by the light microscopy. Tissue from the terminal ileum were grinding into a homogenate, and 4000r/min centrifugal for 10min. The intestinal IL-6 concentration from supernatant was detected by ELLSA method.Results: (1)Clinical reseach: Data showed that the total effective rate of the therapeutic group was96.2%, and the control group was 92.0%. All patients in the therapeutic group successfully completed radiation therapy, while in control group, three patients suspended radiation therapy for two days and four patients for one days because of poor control of diarrhea. During the study, there was 2 patients of nausea in therapeutic group, but no impact on the treatment. (2) Experimental studies: After 4 days, in C group(model control) one rat was dead, others underwent more severe diarrhea, hematochezia and depression than A and B group. By the light microscope, C group had inflammatory cell infiltration, some villous epithelial cell loss and erosion, and lower levels of the height of villus than A and B group. IL-6 concentration was higher in C group than in A, B, and C group(P<0.01).Conclusions: The results of this study suggested Gegenqinlian Modified Decoction was the effective and safe prescripion in the treatment of radiation enterocolitis of damp-heat syndome by significantly improving the cure rate of acute radiation enteritis and a series of symptoms, as well decrease intestinal IL-6 concentration which might have a beneficial effect in the protection intestinal mucosa from radiation damage.
